Joining 2 Days of aesthetics consideration in soft/ hard tissue management presented by Dr. Homa Zadeh. Great to gather again after COVID and his presentations are truly inspiring! Hands-on workshop using new VISTA kit and BoneModels simulation models really allow us to learn the foundation of managing periodontal phenotype therapy accurately.

Peri-implant soft tissue dehiscence/ deficiency (PSTD) has become more prevalent in recent clinical follow-up. A classification of PSTD is presented by Zucchelli G, Tavelli L, Stefanini M et al. discussing single implant sites at esthetic zone.

4 classifications are built by determining the level of the gingival margin of natural tooth, colors, visibility of the abutment/ implant through the mucosa and the thickness of keratinized/ soft tissue.

Similar to gingival recession of natural tooth, varies surgical procedures such as CAF, CAF + CTG, combined therapy and etc. could be selected. Through the classification the clinicians are able to follow a proper treatment protocol to relieve unfavorable outcomes.

An ideal implant position and diameter should be selected carefully according to clinical anatomy. In order to make the restoration both esthetically and functionally successful, biologic width and the amount of buccal bone needs to be highlighted.

Rojas-Vizcaya proposed and reported a prosthetically-guided clinical case using biological aspects as a rule, to correspond the ideal three-dimensional position of the implant, as The 3A-2B Rule, via CBCT.

The treatment plan should determine the occlusal plane or incisal edge prior surgical procedure. Cervical contour and the level of the remaining bone should both be taken into account, allowing the shape of the (provisional) prosthesis to be formed. Using the provisionalization as surgical guide, the implant placement will be able to reach the proper position and angulation.

Root coverage therapy has become highly demanded because of the necessity for esthetic purpose. Both the coverage ratio and the color of the tissue should be taken into account.

Rasperini et al. summarized the contemporary methods of treating gingival recession and made decision trees based on different aspects. (patient/ treatment selection) Varies clinical condition including the morphology of the recession, phenotypes, flap design, recipient sites, quality of CTG and suture techniques effect the outcomes.

It is suggested that each clinical situation should be evaluated carefully in order to fit the appropriate surgical methods and meet the esthetic appearance.

Surgical therapies have become essentials to treat peri-implantitis accurately. Almost a quarter of peri-implantitis exhibits a combination of destructive configuration, making the morphology of wall defects, especially the buccal wall, an important role for the management.

Monje & Schwarz have presented several technical principles to exceed the defects. An aspect of identifying the “reparative potential” is announced, and claimed to predict the regenerative outcomes.

The main goal to treat peri-implanitis is to minimize the colonization of pathogenic flora of the disease. In order to accomplish this propose, achieving stability of the wound is highlighted, ensuring clot formation and healing process of the infected site.

Article digest #7 - Minimizing Discrepancy using Tilted Sloped Implants
For full mouth rehabilitation, as all-on-X protocol, it is highlighted to minimize the discrepancy in the tilted implant (posterior) site. Flat implant platform may cause some problems whether placing the implant in distal subcrestal position or in mesial supracrestal position. Using Sloped implant platform in tilted position may allow the implant reaching the true bone-level position, eliminate the need of bone crest reduction and decrease implant-alveolar bone gap.
