❤የደም ስር መዘጋት እና “ጋንግሪን”❤️
o በሰውነታችን ውስጥ የተጣራ ደም ከልብ ወደ ልዩ ልዩ የሰውነት ክፍሎች የሚያሰራጩ "ደም ቀጂ /አከፋፋይ " ደም ስሮች(arteries) እና ጥቅም ላይ የዋለ ደምን ይጣራ ዘንድ ወደ ሳንባ እና ልብ የሚመልሱ "ደም-መልስ" ደም ስሮች(veins) ይገኛሉ።
o ዋና ዋና ደም አከፋፋይ የደም ስሮች (arteries) ሲዘጉ ፤ በደም ስሩ ምግብና ጠቃሚ ንጥረ ነገሮች ያገኝ የነበረው የሰውነት ክፍል (እጅ፣እግር፣አንጀት ወዘተ...) ሊሞት ይችላል፤ ይሄኔ "ጋንግሪን/gangrene" ሆነ ይባላል።
o በእርግጥ እንደተዘጋው የደም ስር አይነት እና ቦታ ታካሚዎች በ“ስትሮክ”፣ ልብ ድካም፣ በአንጀት መዘጋት ወይም በሌሎች ምልክቶች ወደ ህክምና ተቋም ሊመጡ ይችላሉ።
o የዚህ ፅሁፍ ዓላማ በተለይ በእጅና፡ እግር ላይ የሚከሰት አጣዳፊ የደም ስር መዘጋት በሽታ ምልክቶች እና ህክምናን በተመለከተ በወፍ በረር ለማስተዋወቅ ነው።
o የደም ስሮች በልዩ ልዩ ምክንያት ፤በአጣዳፊነት ወይም ቀስ በቀስ ሊዘጉ ይችላሉ።
ከአጣዳፊ ምክንያቶች መካከል : በድንገተኛ አደጋ(በመኪና፣ በጥይት ፣በጩቤ፣በማሽን አደጋ ወዘተ...) ፣በደም መርጋት ፣ከልብ በሚበተን የረጋ ደም(embolus)፤ ተጠቃሽ ናቸው። ይህንን በመሰለው ችግር ምክንያት የሚመጣ የደም ስር መዘጋት ፣ በአብዛኛው ፋታ የማይሰጥ ነው ። በመሆኑም በአስቸኳይ የደም ስር ህክምና/ቀዶ ህክምና ካልተደረገ በሰዓታት ውስጥ ደም ያጠረው የሰውነት አካል(እጅ፣እግር፣አንጀት ወዘተ...) ወደ መሞት("ጋንግሪን") ሊያመራ ይችላል ።
o ቀስ በቀስ የሚከሰት የደም ስር መጥበብ ቀድሞ የነበራቸው ታካሚዎች ሲሆኑ ከአጣዳፊው ክስተት በፊት ሌሎች ምልክቶች ይኖራቸዋል።ለምሳሌ በሚራመዱበት ጊዜ እግር ህመም (በአብዛኛው ባት እና ታፋ ላይ ) ይሰማል፣ ቆዳ ይደርቃል፣ ጥፍሮች ይጠነክራሉ፣ ወዘተ …ችግሩ እየተባባሰ ሄዶ አስከፊ ደረጃ ላይ ሲደርስ ያለምንም እንቅስቃሴ በተለይም በመኝታ ግዜ የእግር መርገጫ ና ጣቶችን ማቃጠል፣መቁሰልና መሰል ችግሮች ይኖራሉ።
አጣዳፊ የደም ስር መዘጋት ምልክቶች
የእጅ/እግር ደም ስሮች መዘጋት ልዩ ልዩ ምልክቶች ሊያሳይ ይችላል፡ ከነዚህም መካከል :
o የጣቶች ህመም/ማቃጠል፣ መደንዘዝ ፣ ውሃ መቋጠር ጣቶች፣እግርን/ እጅን ማዘዝ (ማንቀሳቀስ) አለመቻል;የእጅ እና እግር መጥቆር እና ፈሳሽ ማምጣት ሊሆን ይችላል
o የእግር የደም ስሮች ሆድ ውስጥ ካለው ትልቀ የደም ስር(aorta)ጀምሮ ድንገት ሲዘጉ የሁለቱም እግሮች አለመታዘዝ ሊያጋጥም ይችላል ::
ህክምናው እንዴት ነው?
· የደም ስር በአጣዳፊነት ተዘግቶ ባለበት ሁኔታ የምትጠፋ እያንዳንዷ ሰዓት የእጅና የእግርን የመትረፍ ዕድል እያመነመነው ይሄዳል(ህክምና ቢሰጥ እንኳ)። አልፎ ተርፎም በህይወትም ላይ ጭምር አደጋ ሊያመጣ ይችላል።
· ታካሚዎች በጊዜ ወደ ህክምና ተቋም ከመጡ - እንደበሽታው መንስኤ የተለያዩ አካል ማዳንን ታሳቢ ያደርጉ ህክምናዎች ሊሠጡ ይችላል:
· መድሃኒት በመስጠት (ደም ማቅጠኛዎችን ጨምሮ እንደ ስኳርና የደም ብዛት ላሉ ሌሎች ተጓዳኝ በሽታዎች)፣ እንዲሁም በቀዶ ህክምና የረጋ ደምን መጥረግ(thromoboembolectomy)፣
o ከአጣዳፊው የደም ስር መዘጋት/ደም መርጋት በፊት የነበረ(የከረመ)ለአጭር ርቀት የተዘጋ/የጠበበ ደም ስርን መጠገን/መለጠፍ(endarterectomy) ሊደርግ ይችላል።
o ከአጣዳፊው የደም ስር መዘጋት/ደም መርጋት በፊት የነበረ(የከረመ)ረጅም የሆነ የደም ሥር መጥበብ/መዘጋ ት በሚያጋጥምበት ግዜ bypass መሥራት ይቻላል

IM injections Need attension
A wrong intramuscular (IM) injection can, in rare but serious cases, lead to paralysis or nerve damage. Here's a breakdown of how this can happen:

1. Nerve Injury

Injecting into or too close to a major nerve — particularly:

Sciatic nerve in the buttock (gluteal region)

Radial nerve in the deltoid region

Femoral or lateral femoral cutaneous nerves in the thigh

Result: Immediate sharp pain, weakness, numbness, or even long-term paralysis in the affected area.

2. Incorrect Site Selection

Using the wrong site (e.g., upper inner quadrant of the gluteal area instead of the upper outer quadrant) increases the risk of hitting a nerve.

3. Improper Technique

Poor technique can cause:

Deep tissue damage

Nerve impingement

Injection too close to bones or blood vessels

4. Toxicity of the Drug

Some medications are not safe if they come in contact with nerve tissue. For example:

Certain antibiotics (e.g., penicillin G)

Irritating substances or oil-based medications

If injected too close to a nerve, they can cause inflammation, demyelination, or necrosis of nerve tissue.

5. Compartment Syndrome or Vascular Compromise

If the injection leads to:

Hematoma

Swelling that compresses nearby nerves

Blocked blood flow to nerves

It can result in temporary or permanent loss of function.

Signs of Injection-Related Nerve Injury:

Burning or shooting pain during injection

Weakness or inability to move the limb

Numbness or tingling

Muscle atrophy (in long-term cases)

What to Do If This Happens:

Immediate medical attention — time is critical.

Neurological evaluation

Imaging (MRI, EMG) to assess nerve damage

Pain management

Physical therapy

Surgery — in severe cases (e.g., nerve repair).

🫂Mpox vaccine🫂
The virus that causes mpox is related to the virus that causes smallpox. JYNNEOS is a 2-dose vaccine developed to protect against mpox and smallpox.
People need to get both doses of the vaccine for the best protection against mpox.
The second dose should be given 4 weeks after the first dose. However, if it has been longer than 4 weeks since you got the first dose, get the 2nd dose as soon as possible.
Whether or not you've been vaccinated, continue to reduce your risk of getting mpox.
The Mpox vaccine is recommended for individuals at higher risk of exposure to the virus, including gay, bis*xual, and other men who have s*x with men GBMSM (forbiden in Ethiopia). Other groups recommended for vaccination include transgender and gender-diverse individuals, s*x workers, people living with HIV (if at risk of exposure), those with s*xual networks that include GBMSM, laboratory personnel working with orthopoxviruses, and healthcare workers at risk of exposure to patients with Mpox. Additionally, pregnant or breastfeeding individuals at risk of exposure are advised to receive the MVA-BN vaccine.

Here's a more detailed breakdown of who should consider the Mpox vaccine:

👉GBMSM: Due to the increased risk of exposure in this population, vaccination is strongly recommended.

👉Other individuals with increased risk: This includes those with s*xual networks that include GBMSM, transgender and gender-diverse people, s*x workers, and people living with HIV who are at risk of exposure.

👉Healthcare workers: Those at risk of exposure to Mpox patients should also consider vaccination.

👉Laboratory personnel: If they work with orthopoxviruses, they should be vaccinated.

👉Pregnant or breastfeeding individuals at risk of exposure: The MVA-BN vaccine is safe and recommended.

የብሔራዊ አደጋ ምላሽ አቅምን ለማጠናከር ትልቅ እርምጃ የሆነው የልምምድ መርሃግብሩ የኢትዮጵያ የሕብረተሰብ ጤና ኢንስቲትዩት (EPHI) ከ UK MED እና UK Emergency Medical Team (UK EMT) ጋር በመተባበር በሀገሪቱ የመጀመሪያውን የተሟላ የድንገተኛ ህክምና የመስክ ልምምድ "የማይበገር የህብረተሰብ ጤና አደጋ አስተዳደር ወደ የተሟላ የልህቀት ማዕከል" በሚል መሪ ሃሳብ ተካሂዷል።

"ይህ ልምምድ ዓለም አቀፍ ደረጃውን የጠበቀ የአደጋ ጊዜ ምላሽ ሥርዓት ለመገንባት በምናደርገው ጉዞ ወሳኝ ምዕራፍ ነው" ሲሉ የጤና ሚኒስትር ዶ/ር መቅደስ ዳባ ገልፀዋል።

የዩናይትድ ኪንግደም መንግስት የአለም የጤና ድርጅት ባወጣው መለኪያ መሰረት ዓይነተ አንድ የአደጋ ጊዜ የህክምና ቡድን (Type 1 Fixed EMT) አለም አቀፍ ደረጃን ለማግኘት የሚያስችሉ ሙሉ ግብዓቶች ድጋፍ የተደረገ ሲሆን፤ በመርሀ ግብሩ የግብአቶች ድጋፍ ብቻ ሳይሆን የዕውቀት ሽግግርም ተደርጎበታል። “ይህ የልምምድ መርሀ ግብር ከኢትዮጵያ ጋር ጠንካራ የጤና ስርዓትን በመገንባት ላይ ያለንን አጋርነት አንዱ ማሳያ ሆኖ ያገለግላል” ያሉት በኢትዮጵያ የእንግሊዝ አምባሳደር ዳረን ዌልች ናቸው።
የተደረገው ድጋፍ እና የልምምድ መርሃ-ግብር የሃገራችንን የድንገተኛ አደጋ ምላሽ አሰጣጥ አንድ እርምጃ ወደፊት ያራመደ መሆኑን የገለጹት የኢትዮጵያ ህብረተሰብ ጤና ኢንስቲትዩት ዋና ዳይሬክተር ዶ/ር መሳይ ኀይሉ ናቸው።
በዝግጅቱ ላይ የፌዴራል ጤና ሚኒስቴር፣ የክልል ጤና ቢሮዎች፣ አለም አቀፍ ኤጀንሲዎች እና ከአፍሪካ የበሽታዎች መቆጣጠር ድርጅትም ተገኝተዋል።
