MY OPINION ON THE MEDCS VS NONMEDICS DEBATE ABOUT AESTHETIC MEDICINE PROCEDURES IN POLAND
Some people say that non-medics should not perform these procedures because they don’t have the knowledge to treat complications and manage side effects. But a doctor also cannot cure certain complications (for example, filler blockage of the artery supplying the eye). Maybe an ophthalmologist could (but rather not in an clinic setting), a GP definitely could not. Firstly, the retina begins to die irreversibly very quickly — minutes matter. After 10 minutes, the risk of massive, irreversible retinal necrosis is already very high. Hyaluronidase may work theoretically, but in practice not necessarily. It may work if the blockage is indirect (for example in branches, not in the main trunk), because hyaluronidase does not directly reach the lumen of the retinal artery. Additionally, the vascular and anatomical barrier of the eye limits its pe*******on. In central retinal artery occlusion, the time is usually already lost.Treatment options to restore vision are very limited, and the prognosis is often poor due to rapid and irreversible retinal damage. A GP cannot handle this. They will not perform retrobulbar injection or selective intra-arterial thrombolysis with hyaluronidase in an clinic setting — which is extremely rare and difficult to access.
Among other reasons, this is why I do not perform filler procedures in the nose or forehead area at VIP AESTHETIC MEDICINE, for ethical reasons, in high-risk areas of vision loss. If aesthetic medicine in Poland is to be exclusively for doctors (not nurses, other medical professionals, or cosmetologists), then please explain to me: what specialty doctor will perform selective thrombolysis with hyaluronidase in their private aesthetic clinic, when this requires interventional neuroradiology? Central retinal artery occlusion affects the inside of the eye and retinal vessels — this belongs to interventional ophthalmology and hospital treatment.
What is happening in Poland is unfair and following this flawed logic only orthopedic surgeon should teach ice skating in case someone breaks a leg, only gynecologist should do intimate waxing..
