Autumn Years Community Care Ltd We are an experienced and established team supporting people living with dementia and those who care for them.

We can support you whether you have received a diagnosis, are worried about memory problems or care for someone living with dementia. Through an innovative range of services we offer the older generation and their caregivers solutions to the problems that they face when confronted with such a life changing diagnosis. Our focus will always be on reducing loneliness and isolation, improving physical and mental wellbeing and helping our clients to maintain their independence and quality of life.

We just wanted to extend a very big THANK YOU to the entire team at the Bristol Dementia Action Alliance for inviting us to last month's disco. We were truly made to feel very special, and we had the most amazing time. A special shoutout to Judy, who dazzled everyone with her dance moves and even won a first prize! No one left empty handed, the retro pick and mix is still a topic of conversation.

Thanks again for such a fantastic afternoon. We genuinely look forward to future events and working alongside you more.

Another amazing day out here at Autumn Years Community Care Ltd… This time at Slimbridge! We were truly blessed with the changeable weather today it was in definitely in our favour.
With the company of great friends, we enjoyed feeding the geese and swans, we watched some incredible bird shows, the playful Otters came over to say hello and we had a delightful picnic sat in the sunshine , We even had a bit of excitement with a cheeky pelican named Spout, who almost had us organising a search party when he flew off in the wrong direction! It’s thanks to the dedication of our wonderful staff, we can plan and enjoy these days together.

A huge shoutout to Faye and Sophie at Severnside Community Rail Partnership for organising a fantastic train trip to Bath Spa! They ensured everyone was included, and we all had a wonderful day. The 'Day Trips By Train' experience was comfortable and thoughtfully planned. Starting from Bristol Parkway, Great Railway's Passenger Assist service was invaluable, helping those who needed assistance to get on and off the trains. We couldn't recommend them highly enough.

Despite the weather being a tad too hot, the bunting above provided much-needed shade as we wandered past the Abbey and found a lovely spot to enjoy a light lunch. We then leisurely made our way back to the station, soaking in the street music and performances. The journey back was equally delightful, with stunning countryside views and a refreshing ice lolly to cool us down.

It was an excellent demonstration of how the railway can cater to accessibility needs. Great Western Railway's Passenger Assist service is there to ensure your journey is as comfortable as possible. Today was truly special, where all centres came together and enjoyed time with each other and what a wonderful time we had at Oakham Treasures! It really would be easier to tell you what we haven’t seen today, than to try and list all the incredible things we did. Every nook and cranny was filled with delightful memories and keys to our childhood. We could have spent hours exploring all the treasures.
One of the highlights was Grace trying to use a rotary telephone - it was so charming and amusing watching Gerald explaining to turn the dial as she pressed the numbers📞
Also Carmel wondering how she would get up onto a penny farthing that was nearly as tall as her.

Please take a moment to enjoy the pictures zoom in and look through the photos and reminisce with your loved ones, both young and old. I'm sure you'll find as much joy in these moments as we did.

Come and join us for an unforgettable time filled with fun, we love being together, working as a team on Art projects, singing, Games and most of all Laughter.

Our Sing for the Brain group the Autumn Years Songbirds! Is held regularly during our sessions at all our days centres, and we would love for you to be a part of them. If you are interested, please get in touch with us.

But wait, there's more! We don't just sing – we are always busy with exciting activities. This month, we need to thank Coniston School for the donation of cakes and some very yummy strawberries. So yummy we did quite get photos of us enjoying them sorry!!
We have been soaking up the sunshine in the community garden watering plants and hoping our seeds will soon pop up, we are not sure what is weeds or flowers at the moment but they are well watered.
But most of all we have been enjoying delicious cool ice creams.

Book your free trial day and come and have a cuppa and enjoy the views!

We just wanted to share some delightful highlights from today's truly special day at Hartcliffe City Farm. We were thrilled to welcome several new additions to our farm and Autumn Years today, and it seems like there might be even more to come!

The sun graced us with its presence, making the day even more enjoyable. We took the opportunity to get out and get an update on all the animals, which was both informative and heartwarming.

Carmel brought us some beautiful flowers from her garden, which we arranged and took home, adding a lovely touch to our day.

The atmosphere was vibrant with music that got everyone up and dancing, and our laughter could be heard over at the play barn. It was a day full of fun, connection, and positive energy.

We are looking forward to more days like this at Hartcliffe City Farm!

If I get dementia, I’d like my family to hang this wish list up on the wall where I live. I want them to remember these things.

1a. Every time you enter the room announce yourself. “Hi Dad - it’s Amanda.”
NEVER ask- Do you know who I am??? That causes anxiety.

1. If I get dementia, I want my friends and family to embrace my reality.

2. If I think my spouse is still alive, or if I think we’re visiting my parents for dinner, let me believe those things. I’ll be much happier for it.

3. If I get dementia, don’t argue with me about what is true for me versus what is true for you.

4. If I get dementia, and I am not sure who you are, do not take it personally. My timeline is confusing to me.

5. If I get dementia, and can no longer use utensils, do not start feeding me. Instead, switch me to a finger-food diet, and see if I can still feed myself.

6. If I get dementia, and I am sad or anxious, hold my hand and listen. Do not tell me that my feelings are unfounded.

7. If I get dementia, I don’t want to be treated like a child. Talk to me like the adult that I am.

8. If I get dementia, I still want to enjoy the things that I’ve always enjoyed. Help me find a way to exercise, read, and visit with friends.

9. If I get dementia, ask me to tell you a story from my past.

10. If I get dementia, and I become agitated, take the time to figure out what is bothering me.

11. If I get dementia, treat me the way that you would want to be treated.

12. If I get dementia, make sure that there are plenty of snacks for me in the house. Even now if I don’t eat I get angry, and if I have dementia, I may have trouble explaining what I need.

13. If I get dementia, don’t talk about me as if I’m not in the room.

14. If I get dementia, don’t feel guilty if you cannot care for me 24 hours a day, 7 days a week. It’s not your fault, and you’ve done your best. Find someone who can help you, or choose a great new place for me to live. With a bus and activities!!

15. If I get dementia, and I live in a dementia care community, please visit me often.

16. If I get dementia, don’t act frustrated if I mix up names, events, or places. Take a deep breath. It’s not my fault.

17. If I get dementia, make sure I always have my favorite music playing within earshot.

18. If I get dementia, and I like to pick up items and carry them around, help me return those items to their original place.

19. If I get dementia, don’t exclude me from parties and family gatherings.

20. If I get dementia, know that I still like receiving hugs or handshakes.

21. If I get dementia, remember that I am still the person you know and love.”
