"Providing rehabilitation as well as preventative healthcare at home is crucial in keeping people active, healthy, and being able to participate and enjoy life." When people are affected by a long term health condition, an injury, or a period in hospital, everyday tasks at home can become difficult. We work with patients in their homes across Bristol and surrounding areas, providing Occupational Therapy assessments and interventions using a person-centred approach. This includes practical advice, rehabilitation, equipment, adaptations, falls prevention, cognitive assessments, support and guidance for carers/ families, and more. We help people to overcome difficulties they are facing with daily tasks, such as accessing the shower safely or reduced confidence in getting out and about in the community. We also work preventatively with those who may not be experiencing specific issues now but would like to be prepared and set-up for the future. This is often especially helpful for those with Dementia and their families.
