Your Social Prescribing Link Worker can connect you with community activities and/or support options to improve your health and wellbeing. They enable people to have more control over their lives, develop skills and give their time to others, through involvement in community groups. They connect people to community groups and help the person to develop skills, friendships and resilience. South Hardwick PCN is made up of 9 GP surgeries:

· The Village Surgery (South Normanton)
· Limes Medical Centre (Alfreton)
· North Wingfield Medical Centre
· Royal Primary Care Clay Cross

Have your say on transport in the East Midlands

The East Midlands Mayor is shaping a new Mayor’s Transport Plan, looking 15 years ahead at how we travel across Derbyshire, Nottinghamshire, Derby and Nottingham.

The plan focuses on three big questions:
• Making buses, trains and trams easier to use
• Helping people get where they need to go without relying on a car, especially in towns and villages
• Taking better care of our roads for driving, cycling and walking

This matters for health, access to services, social connection and the everyday lives of our communities.
