You might typically associate high cholesterol (and particularly LDL) with cardiovascular risk and associate insulin resistance more with risk of diabetes.

In reality, they are closely linked.

Insulin resistance affects far more than blood glucose alone. It influences lipid metabolism, inflammatory pathways and vascular function — all of which contribute to cardiovascular risk over time.

This creates a metabolic environment that not only alters blood markers, but can also affect the structure and function of blood vessels themselves, including damage to the vessel lining and promotion of plaque formation (atherosclerosis).

This is why assessing health purely through cholesterol alone can sometimes miss a big part of the picture.

Dermal filler is not my usual first-line approach when it comes to skin regeneration.

However, there are specific anatomical changes where it can be an appropriate and effective option.

The tear trough is one of those areas.

Over time, structural changes occur between the under-eye region and the upper cheek, often due to separation and descent of the supporting fat pads.

This can create a hollow or shadowed appearance which is not always related to skin quality alone.

In carefully selected individuals, small amounts of dermal filler can be used to support this area and soften the transition, helping to restore a more refreshed and youthful appearance.

I often explain this to my patients as being similar to the tectonic plates of the earth gradually moving apart — in some cases, a small amount of support with the correct dermal filler can help bring balance back to the area.

For safety and little to no bruising, I use a blunt-tip cannula technique in this region and take a conservative, considered approach. Filler is hydrophilic which means as the filler settles into its home, water is drawn to it causing the filler hyaluronic acid molecules to swell. So we always undertreat and review at 2 weeks. I use Teosyal Redensity II as this filler is licensed specifically for the under eye and is clinically proven not to cause tyndall effect; a bluish discolouration seen when dermal filler is placed too superficially under the skin, causing light to scatter through the product.

Tear trough filler is quite long lasting (12-18 months) when placed correctly.

As with all treatments, suitability is determined following full consultation and clinical assessment.

Hormonal change is often discussed in the context of women’s health, but men can also experience a gradual decline in hormone levels over time.

This is sometimes referred to as andropause.

Because changes tend to occur slowly, symptoms are often attributed to stress, lifestyle or ageing rather than recognised as hormonal.

For some men, this may include changes in energy, mood, body composition, libido and cognitive function.

Hormonal decline is widely discussed, particularly in relation to menopause.

However, the concept of “oestrogen dominance” — a relative imbalance between oestrogen and progesterone — is less commonly understood. This can occur when progesterone levels fluctuate or decline in relation to oestrogen, particularly during perimenopause when hormonal patterns become less predictable.

Hormonal wellbeing is influenced not only by the absolute levels of oestrogen and progesterone, but also by the balance between them. For some individuals, this means that even when taking hormone replacement therapy, symptoms may persist if the relative hormonal balance does not feel optimal for their physiology.

As women approach perimenopause, metabolic changes may also play a role. Shifts in insulin sensitivity, body composition and energy regulation can interact with hormonal fluctuations, sometimes making symptoms more complex or more difficult to interpret.

Because these hormones influence multiple systems in the body, symptoms can feel varied and sometimes difficult to connect, such as:

• anxiety or irritability before a period
• feeling overwhelmed or emotionally reactive
• disrupted sleep or early waking
• breast tenderness or fluid retention
• heavier or more erratic cycles

There is significant discussion around GLP-1 and dual GLP-1/GIP medications at present.

Yesterday was our 7 year anniversary at (happy birthday to us!).

Over the years, I’ve come to realise that many of the consultations I conduct are about far more than what someone initially books in for.

A skin concern may be hormonal.

This result reflects progressive healing over several weeks following medical-grade ablative treatment.
