As we step into a new year, I would like to extend my warmest wishes to all my patients and their families. 🌿

May this year bring you health, confidence in your movement, and moments of independence that truly matter.

Working as a physiotherapist with older adults, I see every day how much difference the right support can make — whether it’s improving balance, reducing the risk of falls, recovering after illness or surgery, or simply helping someone feel safer and more confident in their own home/care homes.

Physiotherapy is not just about exercises; it’s about understanding each person’s abilities, fears, and goals, and building strength, stability, and trust step by step, at a pace that feels right for them.

Thank you to the families and carers who are part of this journey — your involvement and support are essential.

🦵 Knee Replacement: When Is It Indicated and What Happens After?

Knee replacement surgery (total knee arthroplasty) can be a life-changing solution when knee pain and stiffness significantly limit daily activities and conservative treatments are no longer effective.

🔹 When is knee replacement indicated?

✔️ Advanced knee osteoarthritis
✔️ Persistent pain, including night pain
✔️ Significant stiffness and reduced range of motion
✔️ Difficulty walking, climbing stairs, or standing up from a chair
✔️ Poor response to physiotherapy, injections, or medication

The decision is made together with an orthopaedic surgeon — but surgery is only one part of the journey.

🔹 What happens after surgery?

🔑 Rehabilitation is essential for a good long-term outcome.

A successful knee replacement is not defined only by the operation itself, but by how well movement, strength, and confidence are restored afterwards.

Rehabilitation is progressive and goal-driven, usually divided into stages:

1️⃣ Early stage (0–2 weeks)
• pain and swelling management
• regaining full knee extension
• early muscle activation
• assisted walking.

2️⃣ Intermediate stage (2–6 weeks)
• increasing range of motion
• building muscle strength
• improving balance and walking quality.

3️⃣ Advanced stage (6–12+ weeks)
• functional strength
• coordination and stability
• return to daily activities with confidence.

🔹 A key message

Exercises are not one-size-fits-all.
They must be adapted to the stage of recovery, individual goals, and lifestyle.

👉 Does the fear of falling make you stop moving?
It may feel like a way to keep yourself safe… but in reality, moving less actually increases the very risk you’re trying to avoid.

What happens when you reduce your activity because of fear?

🦵 Gradual loss of leg strength

⚖️ Reduced balance and coordination

🚶‍♂️ Slower, smaller, more unstable gait

🧠 Lower confidence and higher anxiety

🛏️ Avoiding daily tasks → loss of independence over time

The irony?
The more you protect yourself by saying “I won’t move so I don’t fall,” the more the body adapts to inactivity… and becomes less able to keep you safe.

The good news:
Fall risk decreases significantly with targeted strength, balance and gait-training exercises. Even a few minutes a day can rebuild confidence and control.

Did you know that dizziness and balance problems are not just a normal part of aging?
Around 35% of adults over 40 and up to 80% of people over 80 experience vestibular dysfunction — issues with the inner ear balance system that can lead to:

✅ Frequent dizziness or light-headedness

✅ Difficulty turning the head while walking
