Setting High Standards: How Glyn Rhosyn Helps People Move Forward
When people move on from a service it is more than just a handover of care and support; it is a pivotal moment in their lives. In social care, when someone is ready to move from one service to another, or to their home for life, how we manage that change can make all the difference.
A resident at Glyn Rhosyn who began his journey in secure services, hospitals and care homes over many years and is now preparing for a move to supported living in his home town. This is what he told us:
“I’ve been here a long time and I’m moving on now. I am looking forward to being near to the Anfield Stadium, watching the football, and being close to the town centre. Where I am going to live has a good bedroom, I’ve been to see it. I’ve learnt lots of things at Glyn Rhosyn to help me be more independent.”
Sophie Chamberlain, Registered Manager at Glyn Rhosyn said:
“Relationships with carers, routines, and environments are all part of the foundation that helps people thrive. Whether we visit a prospective resident in their current setting or we work with a new team to support a move along someone’s pathway, we respect that foundation.”
At Glyn Rhosyn, the team values each person’s individual journey, supports their right to make decisions about their care, and ensures they are treated with consistency, respect, and dignity. Plans to move between services or a home for life should be treated with the same care and attention as any other part of someone’s support plan.
Glyn Rhosyn (with nursing) & Glyn Rhosyn Villas (without nursing) is a residential care home for men aged 18 and over with a learning disability or autism, or mental health conditions with associated complex needs.
