If you require travel vaccinations, please kindly complete the Travel Risk Assessment form on our website https://www.elmsmedicalkent.nhs.uk/travel-risk-assessment.

Once you have submitted the form, it will be passed to our nursing team who will be in contact with you in due course.

Please do not use eConsult for travel vaccination requests.
