Case Presentation:
A 58-year-old male presents with a non-healing plantar ulcer for 3 weeks. Despite the size, the patient reports zero pain—a classic sign of Symmetric Peripheral Neuropathy.

Key Data Points:
🔹 HbA1c: 8.9% (Uncontrolled)
🔹 Classification: Wagner Grade 2 (Deep ulcer to tendon/capsule)
🔹 Assessment: Palpable dorsalis pedis but weak posterior tibial pulse.

Management Protocol:
1️⃣ Sharp Debridement: Necessary to convert a chronic wound into an acute wound.
2️⃣ Total Offloading: Using specialized footwear to remove pressure from the ulcer site.
3️⃣ Glycemic Control: Healing is nearly impossible if blood sugar remains >200 mg/dL.

💡 Clinical Pearl: Always perform the "Probe-to-Bone" test. If you can touch bone with a sterile probe, the likelihood of Osteomyelitis is over 85%!

⚠️ Hidden Toxin in Daily Food | Public Health Awareness

Did you know that aflatoxins are toxic substances produced by fungi that can grow on stored food items, especially in warm and humid conditions?

🍚 Common foods at risk
• Groundnuts & peanuts
• Maize / corn
• Rice & wheat stored for long periods
• Dry fruits and spices

🩺 Why is it harmful?
Aflatoxin exposure can affect the liver and, with long-term exposure, may increase the risk of chronic liver disease. Children are especially vulnerable.

✅ How to reduce risk
✔️ Store grains in dry, well-ventilated areas
✔️ Avoid damp, moldy, or discolored food
✔️ Do not consume food with unusual smell or taste
✔️ Prefer fresh stock over long-stored grains

🧠 Medical note:
Prevention through proper food storage and hygiene is the most effective protection.
