Improving Access to Care at Lane End Medical Group

At Lane End Medical Group, we are continually reviewing our services and appointment systems to ensure we provide safe, timely, and effective care.

We recognise that getting an appointment can sometimes be challenging, and like many practices across the country, we are exploring different ways to improve this.

One such model is a Total Triage system. This approach means that every request to contact the surgery begins with an online consultation form. The information you provide helps a senior clinician to review your needs and direct you to the most appropriate professional whether that’s a doctor, nurse, mental health practitioner, physiotherapist, or pharmacist.

For patients who are unable to complete the online form themselves, one of our care coordinators will be available to assist and complete it on their behalf.

The aim is simple: to get you the right care, at the right time, with the right person.

Reviews from other practices already using this system have shown positive outcomes, including quicker access to appropriate care and improved patient satisfaction.

Living Well Taking Control is hosting a free patient webinar on 9th July 2025 from 12:00-12:40 for Alcohol Awareness Week to raise awareness of the shared risk factors between alcohol, Type 2 diabetes, and the importance of early intervention in diabetes prevention.

This session is designed for patients who are at risk of developing Type 2 diabetes or have been diagnosed with prediabetes but have not yet accessed the NHS Diabetes Prevention Programme (NDPP). We are particularly keen to engage patients from deprived areas, ethnic minority backgrounds, younger demographics, and those with a history of gestational diabetes (GDM), as these groups are at higher risk.

Why is this important?

Early intervention can significantly reduce the likelihood of Type 2 diabetes and its complications, improve long-term health outcomes, and reduce strain on healthcare services.

Through this webinar, attendees will:

✔ Learn how alcohol and diabetes are linked
✔ Understand the importance of addressing prediabetes early
✔ Gain practical tips on healthy eating, physical activity, stress management, and better sleep
✔ Discover free NHS resources available to support lifestyle changes

We kindly ask for your support in inviting eligible patients to attend this session. If possible, we would appreciate it if you could send a text campaign to patients who meet the NDPP criteria, encouraging them to register.

WEIGHT LOSS INJECTIONS

We're aware of the growing interest in weight loss medications like Mounjaro. At our surgery, any prescribing of these treatments is done in line with NHS guidelines, ensuring patient safety and effectiveness.

While these medications can be an option for some, they are part of a wider approach to weight management and not suitable for everyone. We encourage you to stay informed by looking at trusted NHS resources online about weight management and the role of these medications. We'll continue to update you on how we manage these discussions in the practice.

Please do not book an appointment with a GP as they will not be able to offer weight loss medication. If you would like to discuss weight management options please book an appointment with one of our health and wellbeing coaches.

What a lovely Coffee Morning we had yesterday! Thank you for all that attended, we saw some new faces who really valued the chance to connect. We had a wonderful Carers Afternoon at Lane End Medical Group. It was fantastic to welcome our carers and speakers from Barnet Carers and Age UK, alongside our amazing Dementia Nurse and Social Prescribing team. We were thrilled be able to provide resources from our local support services. Thank you to everyone who joined us!
