I had a wonderful time teaching at the Basic and Advanced Course in Florida a few weeks ago.

There may be no better course, to teach, learn and be asking the most challenging questions about minimally invasive foot and ankle surgery.

The course was brilliantly organised by A. Holly Johnson Rebecca Cerrato and Oliver Schipper and as faculty, I joined a great team from Chile, Spain, Portugal, France, Brazil, Canada, USA and of course the UK.

Psoriasis is not just a skin condition. As a seronegative arthropathy, it can affect the joints and tendons too. This can lead to hallux valgus (bunions), although the exact mechanism is unclear.

Psoriasis can cause an inflammation of the lining of the joints (a synovitis), leading to joint swelling, pain, damage and deformity. A bunion may be a consequence. As the bunion increases in size, pain on the inside of the foot occurs due to the increase in foot width. The inflamed joint may also be painful due to an arthritic process.

Keyhole surgery for bunion and lesser toe corrections, plays a role in helping with these symptoms in psoriasis. The x-rays in the second carousel image show before, 6 weeks after and 6 months after keyhole bunion surgery in a female patient with psoriatic arthropathy (affecting the joints). At 6 months, you can see the bone remodelling following the bunion correction, with a well corrected foot.

Thankfully, with modern treatment for psoriasis and rheumatoid arthritis, I rarely need to perform surgery in this group of patients.

Delighted to be in Miami, Florida as invited faculty, to the same course I attended when I started my MIS journey over 13 years ago, and looking forward to sharing my experience of thousands of cases of keyhole surgery.

I have the unenviable task on lecturing on ‘Hallux Valgus MIS Complications: how to avoid and deal with them’. The 10 minutes allowed, will be a whistle stop tour!

Thankfully, we have very well defined techniques to reduce complications.

The beauty of keyhole bunion surgery is the power of correction achievable.

The bunion can be moved 100% away from the metatarsal (long bone that connects to the big toe), unachievable with traditional open bunion surgery.

This achieves normal foot alignment, and allows for even severe bunions as shown in these x-rays, to be corrected successfully with low recurrence rates.

This is my dog, Luna, out on a bt of a grey walk recently.

Many of my patients have dogs and love to walk (or have to, to keep the dog exercised). Bunions can have a big impact on this activity by causing pain and limiting walking distance. Even wider, well supportive walking shoes can still lead to painful feet.

Other patients report how other everyday lifestyle choices or physical activities they love doing were either hindered or made impossible because of a painful bunion. From short walks to keeping up with the grandchildren, from dancing to mountain bike rides. My published data following 292 minimally invasive bunion corrections shows that the surgical outcomes ar equally successful regardless of the severity of the bunion.

So, even if your bunion is severe, as in this example, surgery and a successful outcome is still possible!

Having performed minimally invasive (keyhole) bunion surgery for over 10 years, I love seeing patients for follow up, as part of my long term commitment to advancing research in the technique.

These x-rays of the left foot of one of my patients were taken:
🦶 Pre-surgery in May 2018
🦶 Six-weeks post surgery in September 2018
🦶 Seven years post surgery in April 2025

The ability for bone to remodel, stabilising the correction even at 7 years is amazing!
