We give our patients antibiotics in oncology far too easily, often without pause for thought on the benefits vs risks. Inappropriate use of antibiotics breeds multi drug resistance, causes adverse effects which can impact a patient’s quality of life, and this study shows can negatively impact on survival. Antibiotics destroy the gut microbiome leading to bacterial overgrowth of immunosuppressive organisms, causing excess wind and bloating, and as you can see from the meta analysis attached, potentially impacting survival in a negative way. If there is one message I have taken away from it is the importance of optimising the to ensure the most robust immuno surveillance and immune response to cancer neoantigens. We must think twice before “carpet bombing” our patients microbiome with unnecessary anti microbial therapy.

CBM-588 is a probiotic found in the soil in Japan. It’s essentially clostridium butyricum, a bacterium found in a healthy gut . It’s commonly used as a probiotic supplement in . Its effect on amplifying immunotherapy outcomes in kidney cancer in this study is astounding. We have so much to learn about the human microbiome but it’s clear that optimising the microbiome can enhance immune system activity against cancer. This session at on the microbiome was thought provoking and super interesting.

Camu Camu is a purple berry that grows in the Amazon rainforests of Brazil and Peru. It tastes a bit like a sour grape. Also a very potent antioxidant and anti inflammatory “super berry”. It’s also a prebiotic. You can buy it in the local food markets there. How does this relate to cancer? Well here at at a fabulous session all about microbiome optimisation and its effect on cancer outcomes these studies were highlighted. Add camu camu to standard immunotherapy treatment in lung cancer, melanoma or kidney cancer and it appears to “amplify” the effects, leading to superior outcomes. Super interesting. A lot of excitement at ASCO around LAG3, a third checkpoint target (it’s located on T-Cells) following years of PD-L1 and CTLA4 targeted immunotherapy drugs. Relatlimab first in class, great melanoma data. Interested to see how this progresses in the GI space.

Hardly a presentation at goes by without a mention of ctDNA. This has limits of sensitivity. Especially for early stage cancers, and its use for cancer screening (multi cancer early detection) and a lot of work remains to be completed in this space. The advent of new tumour informed ddPCR assays (Signatera, CancerTrack MRD) for MRD detection is a big step forward but still limited in scope for many cancers.
