MIS or Keyhole surgery is especially useful in cases with large deformities (big bunions!) and if there is a significant rotational element to correct (big toe twisted on it’s side)

The alternative here would be a Lapidus operation (fusion of the base joint of the metatarsal) - this is a big operation with significantly long recovery and risk profile.

Are you getting ready for a marathon? With just one week left until race day, it's important to focus on your final week preparations to ensure you're fully prepared for the challenge ahead. From nutrition to rest, there are many important factors to consider. Make sure you're fueling your body with healthy, nutrient-dense foods and staying hydrated throughout the week. Avoid alcohol and prioritize rest and sleep to allow your body to recover from your training. Don't forget about foot hygiene and clipping your toenails to reduce the risk of blisters and other foot injuries. Many runners choose to wear different shoes on race day, but make sure to break them in during your final week preparations to avoid discomfort on race day. There are myths about it that it is painful or unsuccessful but these may relate to historic outcomes of this surgery.

Modern techniques result in reproducible results and great outcomes in most patients.

Pain is generally low or minimal and most patients are surprised at how little pain they are in.

There are no plaster as or any periods off your feet. Patients walk on it immediately and most wear their own shoes within 4 to 6 weeks.

Social media and tabloid press have been keen to portray magic or superior techniques. Often claiming that no fixation is better or getting into a gym the next day after surgery is what we should aspire to.

The truth is fixation provides for a stable osteotomy which improves pain, swelling and healing.

I advise my patients to avoid exercise and too much standing or walking in the early period to allow for the foot to recover and the swelling to settle.

Upper body exercise can of course begin soon after surgery. Low impact lower limb exercise such as cycling is to be delayed to 8 weeks post op and this can be gradually increased to higher impact ones such as running by 12 weeks,

Most patients can wear heels and exercise normally by 3 months.

Sub-specialisation in orthopaedic surgery is well established in elective practice. It is now unusual for surgeons to operate on multiple areas (e.g hip and knee and ankle) and the general orthopaedic surgeon is slowly becoming a thing of the past.

Management of trauma and fractures however is still managed outside of subspecialism structure, but that is also slowly changing.

management is a good example of how subspecialist management of trauma can improve outcomes.

Fixing ankle fractures is not easy and to achieve good results a detailed understanding of the fracture anatomy and biomechanics as well as the surgical anatomy is paramount.

I have witnessed a significant change in how we approach and fix ankle fractures even in that last 5 years led by foot and ankle surgeons. This will undoubtedly improve the outcome for our patients.

Trauma sub-specialisation is an inevitable progression of surgical practice. Better outcomes will follow.

injury is the eponymous name given to injuries affecting the stability of the tarsometatarsal joints of the foot.

Even a so called low energy injury can present with gross instability.

The diagnosis can be difficult and often missed.

This is because ordinary X-rays can be difficult to interpret and specialist knowledge and experience is often required.

Surgery is often indicated to prevent secondary foot collapse and arthritis.

There are various techniques for surgical stabilisation. Bridge plate fixation has significant advantages over other techniques.

With consent

This patient of mine came to see me having been told that his only option is to have his ankle fused.

He had severe arthritis following a fixed fracture over 20 years ago. His ankle was very stiff and had no motion at all.

He is now able to move his ankle β€œfor the first time in 20 years” and even β€œbalance on one leg” which he was able to demonstrate.

He is very happy with the results and so am I.

Ever wondered what a looks like in the flesh?!

This is a good example of a large neuroma, around 8-9 mm in diameter, shown with the stars.

The main interdigital nerve (the nerve that supplies sensation to the neighbouring toes) is shown with the large arrow. This splits into two smaller nerves shown with smaller arrows that reach each toe.

The neuromata normally form at this junction as see here and can get squeezed between the bones causing significant pain.

Surgery to remove this painful lump of tissue can be an instant relief.

A is often treated without surgery as the bones tend to heal quickly.

There are times however that pressure on the head of the metatarsal on walking before it has had a chance to heal can cause the fracture to bend upwards and therefore become relatively shorter.

πŸ‘ŽπŸΌ In this case the second metatarsal fracture had done just that. One can see how the second has become shorter than the neighbouring 3rd.

πŸ‘ŽπŸΌ To make things worse there is also a bunion deformity, the combination had meant excessive over load of the third metatarsal and severe .

😱 Unfortunately as is so common these days this patient had had an ultrasound scan and was told he had a 😰

πŸ‘πŸΌ Treatment was through a corrective osteotomy of the second metatarsal to lengthen it - graft was taken from the patient to fill the gap known as .
