How do you really learn ECG interpretation?

Most of us are told:
“Just see more ECGs.”

But that doesn’t fully explain it.

Even experienced clinicians vary in accuracy, and training often depends on where you happen to work rather than a clear structure.

So what actually helps someone move from beginner to expert?

In this piece, I explore ECG interpretation as a deeper cognitive skill, not just pattern recognition, and question whether our current training approaches reflect that.

A few things to think about:
• Is experience alone enough?
• Why is feedback so variable?
• Why don’t we make expert thinking more visible?

How do doctors actually learn to read ECGs?

We often assume it comes with time and exposure. But that explanation feels incomplete.

Even among experienced clinicians, ECG interpretation isn’t perfectly accurate, and training can vary a lot depending on where and how you learn.

So what’s really driving improvement?

In this British Cardiovascular Society editorial, I explore ECG interpretation as a cognitive skill that goes beyond memorising patterns. It involves physiology, reasoning, and experience, but the way we teach it doesn’t always reflect that.

Some questions I’ve been thinking about:
• Is exposure alone enough to build expertise?
• Why does feedback feel so inconsistent in training?
• How often do we actually see how experts think through ECGs?
• Could training be structured more deliberately?

Interested to hear how others have experienced learning ECGs in different countries, systems or deaneries.
