As promised we have created a new MedLink. Consent for the Pfizer/BioNTech COVID 19 vaccine can only be obtained by a registered healthcare professional. To support practices in gaining informed consent patients can indicate if they:

1 Have read and understood the information
2 Want to proceed to vaccination
3 Decline the vaccine

The patient has the option to receive their completed form by email. They can then print and sign ready for their appointment. The completed form will also come back to the practice at the time of submission and can be automatically coded to the record.

NB. We anticipate the prined copy to be used only on the day to demonstrate consent. There will be no need to scan as the document will already be on the patient record.
