Over and over again I hear patients say things like:

“My veins are so bad there’s nothing you can do with them”
“I won’t be able to have any treatment except surgery because my veins are so bad”

No matter the starting point - there is always something that can be done. It’s not an overnight fix and may take several treatments and multiple visits, but with commitment and patience we can make significant differences.

Varicose veins affect around 60-70% of the population and the majority of these fall into the lower clinical classification of disease meaning that treatment in the NHS is no longer available.

And even if treatment is offered the waiting times currently published are increasing and in some cases are a number of years.

It is estimated that approximately 3% to 6% of people with varicose veins will develop venous ulcers in their lifetime. While many individuals with varicose veins are asymptomatic or only experience minor discomfort, they are at higher risk for developing chronic venous insufficiency (CVI), which can lead to venous leg ulcers if not properly managed.
Key Statistics and Risks:
* Long-Term Risk: Around 3-6% of patients with varicose veins eventually develop these ulcers.
* Skin Changes: Approximately 30% of patients with varicose veins experience skin changes (such as discoloration, swelling, or hardening) linked to chronic venous insufficiency, which increases the likelihood of ulceration.
* Prevalence: Venous leg ulcers affect roughly 1% of the population, but are the cause of over 75% of all chronic leg ulceration.
* Recurrence: Once a venous ulcer develops, up to 70% of individuals may experience recurrence, emphasizing the need for early treatment.

Venous ulcers are considered the most severe manifestation of chronic venous disease, typically resulting from prolonged, high-pressure blood flow (venous hypertension).

The Silent Crisis of Venous Care in the UK

Better access to cost effective treatments would improve many lives. High quality evidence clearly supports the treatment of common venous diseases. Treatment of symptomatic varicose veins improves quality of life and is highly cost effective while early treatment of venous leg ulcers speeds healing, reduces recurrence, and potentially reduces costs. Despite this strong evidence, and National Institute for Health and Care Excellence (NICE) guidance recommending referral of people with symptomatic primary or recurrent varicose veins, access to treatment for symptomatic varicose veins is restricted in much of the UK, and people who develop leg ulcers experience delay in getting the treatment they need for underlying venous disease. Preventing and treating leg ulceration is perhaps the most pressing issue. The importance of early referral and specialist treatment was highlighted in the 2019 report of the All Party Parliamentary Committee on Vascular and Venous Disease, Venous Leg Ulcers: a Silent Crisis.

This is taken from an article that was published this week in The British Medical Journal and was authored by two eminent UK vascular surgeons.
