針對現時英國的腦膜炎爆發，我給您以下建議：

1. 了解症狀，因為病情發展迅速，可能危及生命。

症狀包括：劇烈頭痛、發燒、畏光、頸部僵硬、嘔吐、手腳冰冷、意識混亂和嗜睡。
如果您或您認識的任何人出現上述任何症狀，請立即就醫，聯絡您的家庭醫生、撥打NHS 111熱線或在緊急情況下撥打999。

2. 檢查您的孩子是否曾接種過腦膜炎ACWY疫苗。如果他們在英國 Year 9之後入學且錯過了接種，您可以在他們25歲之前諮詢您的家庭醫生，讓他們接種。

3. B型腦膜炎疫苗目前可以自費接種，但供應短缺。
腦膜炎仍然是一種罕見疾病，如果您不屬於高風險族群，感染風險仍然很小。
注意個人衛生，避免共用接觸唾液和黏液的物品，並經常洗手。

如果您的孩子在2015年後出生於英國，並且在嬰兒時期接種過疫苗，您可能需要考慮在他們上大學前為他們接種加強劑。

4. 如果您收到英國衛生服務管理局 (UKHSA) 的建議或聯繫，請服用預防性抗生素。

*腦膜炎疫苗不屬於香港兒童免疫計畫的一部分
*在英國，嬰兒自2015年起接種B型腦膜炎疫苗，13-14歲（Year 9）的兒童可接種ACWY腦膜炎疫苗，25歲以下的兒童可補種。

由於腦膜炎病毒株不同，B型腦膜炎疫苗無法預防其他株，ACWY腦膜炎疫苗也無法預防所有腦膜炎。

疫苗需要1-2週才能生效。
預防性抗生素可以阻止病毒傳播。
目前，已有超過2500名符合條件的人群和密切接觸者服用了預防性抗生素。
肯特大學正在為學生進行定向疫苗接種計劃。

My advice to you with the current outbreak :

1. Know the symptoms, as it can develop quickly and can be life threatening.

Severe headache, fever, light sensitivity, stiff neck, vomiting, cold hands, feet, confusion and drowsiness.

If you or anyone you know develops any of these symptoms, seek medical help immediately by contacting a GP, calling NHS 111 or dialling 999 in an emergency.

2. Check if your child had their Meningitis ACWY vaccine. If they started school in UK after year 9 and missed it, you can ask your GP for this before they turn 25 years old.

3. The meningitis B vaccine is available privately but there is a shortage at the moment.
It is still a rare disease and if you are not in a high risk group the risk is still very small.
Take precautions with hygiene and avoid sharing items that involves saliva and mucus, wash your hands regularly.

If your child was born in UK after 2015 and had the vaccine as a baby, you may wish to consider giving them a booster before they go to university

4. Take prophylactic antibiotic if you are advised or contacted UKHSA

Meningitis vaccine is not part of Hong Kong childhood vaccination programme

In UK, babies are vaccinated for meningitis B since 2015, and Meningitis ACWY vaccine is offered to children age 13-14 ( year 9) and catch up dose up to age 25years

Because there are different strains of meningitis the MenB vaccine does not protect against other strains and MenACWY does not protect against Meningitis
Vaccine will take 1-2 weeks to work.

當你在英國關節疼痛時，你知道該何時就醫嗎？
關節疼痛可能由多種原因引起。

以下是一些常見原因：
1) 退化性關節疾病，如骨關節炎。
- 俗稱關節「磨損」。膝關節和髖關節等部位經常受到影響。

通常透過調整生活方式、服用止痛藥和物理治療來控制疼痛。如果疼痛影響到您的日常生活和活動能力，請就醫。

2) 自體免疫疾病，例如類風濕性關節炎。
- 常影響手、腳、腕等較小的關節。關節會發熱、僵硬和腫脹。

就醫並專介至風濕病專科醫生進行早期診斷非常重要，這樣才能儘早開始治療，防止關節發炎進一步加重。

3) 晶體性關節病，例如痛風。
- 常影響單一關節，伴隨急性發作，常見於大腳趾，但也可能發生在任何關節。急性痛風有處方藥。

如果您突然出現劇烈疼痛、發燒、腫脹且無法活動的關節，並伴隨發燒，這可能是關節感染，您應該前往急診室就診。這種情況通常會影響肩關節、髖關節或膝關節等部位。
雖然這種情況並不常見，但需要住院治療。

最後，如果您在跌倒等受傷後出現關節疼痛，最好前往Minor Injuries Unit 或急診室，因為很可能需要進行 X 光檢查。
在某些地區，您需要撥打 NHS 111 熱線才能轉診至Minor Injuries Unit，因為他們不提供Walk in就診服務。

Do you know when to seek help when you have joint pains?
Joint pains can have a number of causes.
Here are some common ones

1) Degenerative joint disease such as osteoarthritis.
-Commonly known as " wear and tear" of the joints. It is often joints such as knees and hips that are affected.
It is generally managed with lifestyle, pain medications, and physiotherapy. See a doctor if pain is affecting your daily life and function.

2) Autoimmune conditions such as Rheumatoid arthritis.
-Often affects the smaller joints of the hands, feet, wrists. The joints become hot, stiff and swollen.
It is important to see the doctor to be referred to a rheumatologist for an early diagnosis so treatment can be started to prevent further joint inflammations.

3) Crystal arthropathy such gout.
-Often affects a single joint with a flare up, commonly affects the big toe but can be any joint. There are medications on prescription for acute gout.

If you suddenly develop a very painful, hot and swollen joint which cannot move, and associated with fevers, this could be an infected joint, which you should attend A&E. These usually affects joints such as the shoulder, hip or knee.
Although this is not common, it will need hospital treatment.

Finally if you are concerned about joint pains after an injury such as a fall, it is best to go to Minor Injuries Unit or A&E as an xray is most likely needed.
In some areas, you need to call NHS 111 to be referred to the Minor Injuries Unit as they do not offer a walk in service.

如果您懷疑自己患有痛風，最好諮詢醫生。

我們可以確診並開立口服藥物治療急性痛風。同時，評估是否存在其他潛在病因也至關重要。

我們可以為您提供生活方式建議，並在必要時提供降尿酸治療。

在英國，除非由專科醫生開立處方，否則注射治療並不常見。我也不建議在沒有醫生指導的情況下反覆自行治療。

許多人沒有意識到血液中尿酸升高可能與其他疾病（例如心血管疾病）有關，因此妥善管理尿酸水平非常重要。

If you think you have gout, it is best to consult a doctor.
We can confirm the diagnosis and prescribe oral medications for acute gout. It is also important to assess if there are other underlying causes for gout.
We can give you lifestyle advice and offer uric acid lowering therapy if needed.

Injections are not common in UK unless initiated by specialists. I also do not recommend repeatedly self treating the condition without a doctors guidance.

Many people do not realise raised uric acid in the blood can be linked to other conditions such as cardiovascular disease, so it is important to manage the condition properly.

Look after your mental health

沒有了免費廣東話輔導之後

BNO Visa 計劃的第5年，政府沒有再資助機構提供免費的廣東話輔導或治療服務。移英家庭如有情緒困擾，就要在社區尋找了。

能以英文為媒介的成人，可考慮自行申請或由GP轉介NHS Talking therapies服務。情況若需要藥物或更多醫療專業支援，就要可要求GP 跟進或轉介其他成人精神健康服務（Adult Mental Health Services)。社區內也有地區團體為成人提供輔導/治療服務，如Mind等。
青少年及兒童服務方面，學校的支援更形重要。家長或會不知從何求助，因為英國學校絕大部份不如香港般會有駐校社工，但部份學校會設有ELSA（Emotional Literacy Support Assistant）、駐校護士(School nurse)或輔導員(School Counsellor)一職，部份或會有與社區機構合作的支援計劃。要了解學校可提供什麼支援，在學校網頁找尋Mental Wellbeing 或 Pastoral Care，先行了解當中的支援。若資料不多，就直接聯絡Head of Year 或Mental Wellbeing / Pastoral Care Team查詢校內可提供的協助。

自 18/19 年起，英國國民保健服務(NHS)在英格蘭的學校開設「心理健康支援團隊」（Mental Health Support Teams，簡稱 MHSTs），以協助學校職員及早辨識輕度至中度心理健康問題的學生。截至2025年春季，覆蓋範圍已達到學生總人口的 52%。該計劃是由CAMHS(兒童與青少年心理健康服務)的專業人員透過早期介入如小組及個別輔導，協助那些未達到CAMHS評估門檻的學生早日走出情緒困局。若想知道學校是否受惠於此計劃，可在網絡打MHST及所住區份。要留意申請使用MHSTs的服務，必須由學校轉介，所以家長及年青人在聯絡校方時也可以詢問能否轉介至MHSTs跟進。

廣東話輔導或治療服務方面，只餘下付費的服務。英國心泉為移英的兒童及青少年提供資助，讓孩子可以低於市場價格接受精神健康初步評估及最多8節的輔導/治療。家長及年青人可以參閱我們的網頁https://hongkongwell.uk/subsidized-counselling-service/，了解申請條件及服務範圍。若是成人，大家可在專業人員平台如www.psychologytoday.com 或一些香港人組織如Hong Kong Counsellor Hub 尋持合適的私人執業的專業人員。

最後一提，若有危急情況，打111可轉駁至24小時的危機處理小組(Crisis Team)或到急症室或要求GP緊急會診，這些NHS服務也可要求廣東話翻譯。當然若情況危急就要打999

移民適應是漫長的，大家也要學懂”先照顧情緒，後處理事情”。

您在NHS使用翻譯服務方面有什麼經驗？

最近，我需要為我的NHS 病人使用電話翻譯服務，這些病人分別只說阿拉伯語、尼泊爾語、波蘭語和庫德語等等。
我經常同一時間使用google translate。

從我角度來說 ，遇到的一些實際困難包括：

1. 時間限制導致難以清楚解釋病情，也經常沒有足夠的時間確認病人是否理解我的講解

2. 難以與病人建立關係

3. 不確定翻譯是否準確

4. 難以處理複雜、太私人或情緒化的問題

5. 總感覺病人還有更多問題想問， 整個過程缺乏人情味和滿足感

我最後只能對著電話說話，加上手勢和google translate翻譯，希望能在短短10分鐘的諮詢時間內為病人提供所需的幫助。

所以，能夠有機會用廣東話和我NHS 的經驗為英國的香港人提供醫療援助，我感到非常榮幸。

生蛇和水痘是由同一種病毒－帶狀疱疹病毒（Varicella zoster virus）引起的。

感染水痘或接種疫苗後，病毒會潛伏在人體的神經系統中。
當免疫系統低下時，病毒就有可能被激活。
它會沿著某一條神經線傳播， 並在皮膚上引起 紅疹 ， 俗稱生蛇。

生蛇會傳染嗎？

你不會將帶狀皰疹 ”生蛇 ”傳染給別人。
但是，其他人可能會從帶狀皰疹中感染水痘。

如果你生蛇：
避免接觸免疫系統較弱的人、老年人和新生嬰兒 。
避免接觸孕婦。
保持患處遮蓋。
因為病毒是透過水皰中的液體傳播的。

生蛇建議不上班。
當水皰乾燥結痂後，你就不再具有傳染性了。

Shingles and chickenpox are caused by the same virus ,Varicella zoster virus.
After chickenpox infection or vaccine, the virus will stay dormant in the body's nervous system
When our immune system is low, the virus can be 'reactivated'
It will travel along a nerve and cause shingles on the skin.

Are shingles contagious ?

You cannot pass shingles to someone else
But
Someone can catch chickenpox from shingles

If you have shingles :
Avoid people with weak immune system, elderly, newborn
Avoid pregnant women

Keep it covered
As the virus is transmitted through the fluid in the blisters

Having shingles usually means staying off work

When blisters have dried and scabbed over you are no longer contagious.
