The PEACE & LOVE principle is a modern approach to managing soft tissue injuries. It emphasises a more active and holistic approach to recovery. It is more than symptom control; RICE focused on short-term relief. PEACE & LOVE encourages long-term healing through active recovery. The latest research suggests that early movement, patient education and psychological factors all contribute significantly to recovery outcomes.
“Immediately after a soft tissue injury, do no harm and let PEACE guide your approach.”
PROTECT. Avoid movements and activities that increase pain. This prevents further tissue damage. Protect does not mean rest, as prolonged rest can compromise tissue strength and quality.
ELEVATE. Raise the injured limb higher than the heart to encourage fluid drain and reduce swelling.
AVOID ANTI-INFLAMMATORIES. Contrary to popular belief, early use of ice and NSAIDs such as Ibruprofen may hinder healing by reducing the body’s natural inflammatory response, which is vital for tissue repair.
COMPRESSION. Apply pressure with tape or bandages to reduce swelling and provide support to the injured area.
EDUCATE. Understanding what the injury is, expected healing times and what to expect, can help to follow the correct approach to optimise healing time.
“After the first few days have passed, soft tissues need LOVE.”
LOAD. As pain reduces and allows, gradually reintroduce movement and load to the injured tissue. This promotes tissue repair, strength and resilience.
OPTIMISM. Mental outlook matters. Positive beliefs, confidence in recovery, and avoiding catastrophising are linked to better healing and pain outcomes.
VASCULARISATION. Start gentle cardiovascular activities as soon as possible. This promotes circulation and healing without stressing the injured area.
EXERCISE. Rehabilitation exercises help restore mobility, strength and function. A specific plan from a trained professional supports a safe return to activity and reduces reinjury risk.
