Cardiovascular (CVD) Risk Assessment

Background

In Scotland, Cardiovascular disease is the second largest cause of overall disease burden in our country, and the second most common cause of death after cancer. It causes more than 25% of all deaths in Scotland, many of which are in the under 75 years category and considered avoidable deaths.

This means that they are preventable or treatable through public health interventions or timely and effective healthcare.

To address this issue, the Scottish Government have commenced a national CVD Risk Factors Programme. Their aim is to reduce avoidable CVD deaths by 20% in 20 years.

What we are doing

Auchtermuchty Health Centre is taking part in a new national programme focusing on improving identification and management of clinical risk factors for Cardiovascular Disease.

We are contacting a small group of our patients aged between the ages of 35 – 60, who meet the criteria, to be invited for this health check.

As part of this appointment the clinician will check:

Blood Pressure
Bloods for Cholesterol & Diabetes
Smoking Status
Weight and Waist Circumference

You will be given an information pack and lifestyle advice at your appointment.

Using the blood results and information taken at your appointment a risk assessment will be carried out to assess your risk of developing a problem with your heart in the future.

We will invite eligible patients to our Heart Health Clinic, by sending a text message, telephoning, or inviting them by letter.

We would encourage you to respond to any invitation received.

Initially we will target a smaller group of patients but this may be rolled out to more patients in the coming years.

PATIENT NOTICE - TEST RESULTS

We have received a few queries regarding our text messaging service.

We would like to clarify that we often send text messages to patients inviting them to book a routine appointment with the clinical care team to discuss their blood or test results.

Please rest assured, these messages are non-urgent, and all results are triaged by a clinician.

While we aim to contact patients promptly, it may take up to a few weeks to schedule a consultation - We perform a huge number of blood tests each week, and each result is carefully reviewed by a member of our clinical team. We kindly ask that you remain realistic about the anticipated timeline for review appointments.

Due to our system, some patients may receive a text message only days after their blood samples have been taken. There is no need to be alarmed. Please be assured that if we receive a result that needs urgent attention we will contact you directly, usually by phone.

If you are asked to consult regarding routine blood tests, it generally means the results are not concerning and don't require urgent action. An appointment will give the clinician the opportunity to discuss the results with you in more detail, potentially explain further steps or treatment options, or provide additional information.

In essence, a request for a non-urgent consultation about blood tests is a standard part of healthcare, allowing for a more in-depth discussion of your results and any further necessary steps.

We ask for your patience - we understand it may be taking longer than usual to access any routine appointments in general. This is as a direct result of current long-term sickness, redundancy and annual leave within the clinical team.

As always, we would like to thank those patients who have been exceptionally understanding, and offered kind words of support over the last couple of months.

Please note the practice will be taking part in protected learning time tomorrow. The practice will be closed between 13:00-18:00.

Some GP practices are taking part in a Protected Learning Time session on Wednesday 18 June between 1pm and 6pm.

Practices take part in a number of sessions throughout the year to allow staff to access training. During this time:

▶️ those with appointments should attend unless advised otherwise
▶️ pharmacies should call: 01592 729250 for urgent enquiries

Access to urgent care for patients of surgeries taking part will continue throughout this time.

We would like to take this opportunity to remind patients of the importance of attending their scheduled Physiotherapist and Mental Health Clinic appointments.

Please be advised that a significant number of appointments are being missed, which results in the inefficient use of available resources, and increases patient waiting times. As our appointments are limited and capacity constraints exist, missed appointments cannot be recovered, meaning the appointment cannot be utilised by another patient in need.

To assist us in providing timely care to all our patients, we kindly request that you notify us with a minimum of 24 hours notice should you need to cancel or reschedule your appointment.

We are very fortunate that we have these services available at the practice, reducing the need for patients to travel, however, attendance to appointments is necessary to ensure the Health Board continues to offer the same level of service provision. We would be very disappointed if service provision was reduced or withdrawn.

Your cooperation is greatly appreciated and essential in maintaining the quality and efficiency of our services.

WHY SOME PATIENTS ARE STRUGGLING TO GET A GP APPOINTMENT!!

When you are unwell or need support managing a condition, we know that you want an appointment with a GP or member of the practice team as quickly as possible and with a minimum of fuss. But, too often, that is something that practices struggle to provide.

If it often feels like you are in competition with hundreds of other people to get through to us and secure an appointment, that is because it is quite literally true.

We absolutely appreciate that this is hugely frustrating at times, but practices and all our staff are working as hard as we can. We simply do not have the resources, or sufficient GPs to do all that is asked of us. And the explanation for that is the failure of the Scottish Government to invest in and recruit and retain the GPs you….and Scotland need.

While the number of NHS consultants working in Scotland has increased by 30% in the past 12 years as the health needs of the population have increased, the number of GPs has fallen by 6%.

In that time the number of patients registered with a GP has increased by nearly 9%. As a result of this, while there was one GP for every 1,515 patients in 2013 there is now only one GP for every 1,735 patients. Simply put, there are not enough GPs to provide the level of access to General Practice that patients were previously able to benefit from.

Worse than that, despite the big increases in hospital staff numbers, patients are waiting longer than ever for treatment and therefore need more GP appointments and support while they are waiting. Despite General Practice in Scotland providing over 650,000 appointments every week, it is simply not enough to meet demand.

Poor access to General Practice is frustrating for GPs who want to deliver the best possible service and it is frustrating for patients who feel they are struggling to be seen when they need it.

The majority of Scotland's 898 General Practices are run as independent businesses. They are mainly funded with a fixed income, no matter how much work is asked of them to look after the local community. With that fixed pot they must pay for all their staff, upkeep their buildings and meet all their running costs. General Practice is recognised as being the most efficient part of the NHS in the UK, but a lack of sufficient investment has resulted in where we are today.

Ultimately this is a result of funding decisions taken by Scotland’s politicians and will only truly be addressed with sufficient investment directly into General Practice.

In 2008, the NHS spent around 11% of its whole budget on General Practice. Today, that figure is less than 6.5%.

Practices have also faced significant additional financial turbulence in recent years, and the recent increase to Employer National Insurance Contributions as a result of the 2024 budget at Westminster will add further bills of tens of thousands of pounds to every practice.

Without funding from Government to meet this cost, the only choice that many practices will have to safeguard the future of your GP Surgery is to cut services further. Only recently the partners at Auchtermuchty Health Centre had to make the difficult decision to make one of our Clinical Care Team redundant. Given the huge increase in patient demand each day, it seems almost perverse that we have had to make such a difficult decision that will directly impact on patient access. This decision was not taken lightly, and it is one of the hardest choices we have faced as partners.

Contacting your MSP: how you as a patient can support your local GP practice

The problems in General Practice can be fixed, but it requires a commitment from politicians of all parties to rebalance investment in the NHS so that General Practice receives a proportion of funding in line with the way things used to be.

With proper investment, we can radically improve your ability to see a member of our clinical care team and can do so much more to deliver high quality health services in our local community. We want, like you do, to see GP delivered care, with continuity and quality at the heart. Free healthcare for all cannot exist without an efficient and functioning General Practice. Help us protect the core principles of the NHS.

If you want to see that protection and want to improve our services, please consider taking the time to contact your MSPs and make clear that you believe more investment needs to go to General Practice as a priority.

By working together we can look to a healthier future.
