Margaret - Social Prescriber Social Prescribing explained:
If you have a problem that is not medical, social prescribing is a way of helping you to connect to support.

It can help you to find ways to do something about it and to feel more in control and part of your community.

We’re Short on Drivers — Could You Spare an Hour or Two here at Alsager Community Support

Every Tuesday and Thursday, we help local people get to the Civic lunch club — a lifeline for many who can’t get there on their own. It’s more than just a meal. It’s laughter, conversation, old friends reunited and new friendships made.

Right now, we’re short on volunteer drivers. Two of our brilliant team have had to step back temporarily, which means other volunteers are covering most of the shifts. They’ve been amazing — but it’s a lot. And it’s not sustainable long-term.

So if you live in or near Alsager, have a bit of free time during the week, and like the idea of helping people connect (and meeting some great people yourself), we’d love to hear from you.

Did you know that we are part of the Newcastle North PCN?
The NHS was created in 1948 and over time, the population has grown and more people are living longer. Many people are living with long term health conditions like heart disease and diabetes, many also suffer with mental health issues and may need to access their local health services more often.
To meet these needs, GP practices are working together with community social care, mental health practitioners, pharmacies, hospital, and voluntary services within their areas in groups of practices known as primary care networks (PCNs).

PCNs build on existing primary care services and enable greater provision of personalised, proactive, coordinated and more integrated health and social care for patients.
Newcastle North PCN Ltd brings together 5 GP practices across the North Staffordshire area. We have a range of staff to provide extra support to our practices & patients such as our Clinical Pharmacists, Pharmacy Technicians, Dietician, Paramedic, Mental Health Practitioners, Pharmacists, Social Prescribers, Physiotherapist and Care Home Team.

Why not head over to their website to find out a little more about the team & what they do? https://www.newcastlenorthpcn.nhs.uk/
