Patients attending Newland Health Centre for appointments with Heart of Lincoln Medical Group (Portland/University/Newark rd), and The Brayford Medical Practice, please leave home earlier to get to your appointment on time. Major roadworks are now underway along Newland, and will be in place for the next 7 months.

What is a Primary Care Network ( PCN )

* A PCN is a group of GP practices ( The Brayford Medical Practice & Heart of Lincoln Medical Group) working together with local healthcare care providers
* We collaborate together in delivering national health initiatives and addressing local health needs
* We aim to provide integrated and coordinated care to our local population
* We offer extended access to primary care services, including evening and weekend appointments
* We have dedicated staff such as Pharmacists, Physiotherapists, Mental Health Practitioners, and Social Prescribers to support patient care
