Little toenails are often thick when the others aren’t. Curly or twisted little toes are common and the nail is impacted more than it should be - from either the shoe or from sitting under the adjacent toe.

CHILBLAINS
Chilblains are lumps on the skin (usually red or purplish) which come up as an abnormal reaction to the cold. They can occur when skin is warmed up too quickly after it has been cold. Chilblains usually go away of their own accord within a week or two if further exposure to the cold is avoided.

Chilblains can develop on:
• Fingers
• Toes
• Earlobes
• Nose
• Cheeks
• Heels
• Shins
• Thighs
• Hips

PREVENTION
Keep as warm and dry as possible in the cold. Wearing several loose layers is ideal to trap body heat. After being in the cold, do not heat the skin quickly i.e. putting your hands or feet on a hot radiator or applying a hot water bottle. Warm yourself up gradually. Do not smoke or have drinks that have caffeine in them.

See a GP if your skin hasn’t got any better after 2-3 weeks; there is pus coming out of your skin; you feel hot and shivery or have a very high temperature.

A pharmacist can advise on the best painkiller to take and creams that can soothe the itching.

ARE YOUR SHOES TOO TIGHT?

Common problems caused by ill-fitting shoes:
• Corns/calluses
• Ingrown toenails
• Bunions
• Neuromas (nerve pain/numbness)
• Bruised toenails
• Hammer toes/crossover toes

Many of these problems can be prevented by simply buying the right sized shoe.

Ideally you should be able to fit the width of one index finger between the ankle and the heel. At the opposite end, toes should be able to wiggle freely and one thumb width should fit between the top of the biggest toe and the end of the shoe.

What is a Foot Health Practitioner?..

As a Foot Health Practitioner (FHP) I specialize in the lower limbs and feet with minor ailments recognized and treated. My work as a FHP is non-invasive. External minor surgery without injected analgesia for the treatment of ingrowing toenails or corn removal is typical of the procedures I carry out. Assessment of foot positioning on walking, along with other external examination techniques are used in the assessment of the structure and condition of the lower limb and foot, all of which fall within the ability and remit of the FHP. Thus the work carried out by an FHP far exceed the abilities of those trained simply to provide a nail-cutting service.

In many cases, there is a need for routine maintenance for diabetic patients, patients with arthritic hands, eye troubles, obesity, pregnancy and other debilitating conditions. Many patients will simply have difficulty reaching their feet for routine maintenance - others appreciate the help of a practitioner who they can trust to trim and maintain their nails properly. As a trained observer, the FHP contributes to the health of their patients by screening and alerting other practitioners to indicated diabetes mellitus, malignancies, potential ulceration and other conditions requiring onward referral. Ethics consistent with the medical professions are held, recognised and respected.

Knowledge of the patient’s medical status is taken into account in order to execute the work safely and cause no medical complication. Hygiene, instrument skills, communication skills and a good measure of dexterity are important to the task, as are record keeping and regular updating. Dressing, padding and offloading skills are essential, as is the ability to manage minor wounds. Foot Health Practitioners undertake Continuing Professional Development to retain currency, update and expansion of their skills.
