So you're working on an acute medical ward and a nurse rushes into the doctors office. She tells you her patient is scoring NEWS 8. They are being treated for a Community Acquired pneumonia and were slowly improving however had spiked a temperature, became more confused and desaturated despite being on 15L oxygen. Your perform an Arterial Blood Gas and find the patient to be in Type 1 Respiratory failure. You immediately call your registrar and explain what has happened and the most recent ABG result. They ask you to call the ITU reg for an urgent review and possible transfer.

You pick up the phone, hands shaking after seeing the state of the patient, and nervous to talk to the ITU reg...

The above scenario is an example of the kinds of situation you can expect to be in as a junior doctor. Hospitals are shortly staffed and so it could be that you are presented with a similar situation even if it is your first few weeks of work. Clinical Communication is an important part of medical practice and good communication can make all the difference to patients and to staff. Medical school placements do not prepare us for this.
