Weymouth & Portland Primary Care Network

Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as diabetes and heart disease, or suffer with mental health issues and may need to access their local health services more often.

To meet these needs, we have been working together and with community, mental health, social care, pharmacy, hospital and voluntary services in our local area: Weymouth and Portland Primary Care Network will build on the core of current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and community we serve.

The main aim of Weymouth and Portland Primary Care Network is to focus the services available around local communities, and to further connect the primary healthcare teams to provide workload support for practices at the same time.

Weymouth and Portland Primary Care Network is based on our six practices, serving our community of around 76,000 people. This should be small enough to provide the personal care valued by both patients and GPs, but large enough to have impact and economies of scale through better collaboration between practices and others in the local health and social care system. Our Clinical Director is Dr Tanya Stead who is supported by our Federation Chair Dr Rupert Turberville-Smith. Primary Care Networks are emerging around the country but Dorset has been leading the way in this for some time; we are already making a difference to staff and patients.
