From postoperative till preoperative
𝗥𝗖𝗧, #𝟑𝟔 𝐰𝐢𝐭𝐡 𝐃𝐌𝐄, 𝐌𝐁,𝐌𝐋 ( 𝐜𝐮𝐫𝐯𝐞𝐝 ) & 𝐃𝐁 🦷👨‍🎨

A 22-year-old male presented with severe pain in the lower left jaw. The pain started spontaneously without any clear trigger and gradually became constant. It was intense, sharp, and worse at night, preventing the patient from sleeping. Hot and cold stimuli increased the pain, and the discomfort persisted even after the stimulus was removed. Chewing on the left side made the pain stronger.

After a few days, the pain began to radiate toward the ear and jaw, making it difficult for the patient to identify the exact tooth at first. Swelling then appeared in the area, followed by a bad taste in the mouth. The patient later noticed pus discharge near the tooth. Analgesics provided only temporary relief, with the pain returning more severe each time.

Clinical examination confirmed irreversible pulpitis with acute infection in tooth 36, the lower left first molar. This case highlights how untreated dental pain can quickly progress from pulp inflammation to active infection.

Patient Overview
Age/Gender: 18-year-old female
Chief Complaint: Pain in the lower left jaw, especially on cold and chewing
Tooth involved: #37 (confirmed via periapical X-ray: #36 extracted, no #38 present)
Medical History
Asthma: Controlled with inhaler — avoid NSAIDs or aspirin
Gastroparesis: Delayed gastric emptying — avoid GI-irritating meds
Bad habit: Frequent ice chewing (possible sign of iron deficiency/pica, contributing to enamel trauma)
Clinical & Radiographic Findings

Deep carious lesion on #37, approximating the pulp
No periapical radiolucency
Lamina dura intact
Cold sensitivity present, but pain does not linger
No spontaneous pain, no pain to hot, no night pain
No swelling or sinus tract
Tooth is restorable

📈 Prognosis

Excellent, provided the irritant (caries) is removed and a proper seal is placed.
If treated promptly, the pulp can return to normal with no further intervention.
🦷 Treatment Plan

Caries removal under isolation
Selective caries removal (if needed, for deep areas)
Indirect pulp capping or pulp protection using calcium hydroxide or bioceramic liner
Definitive restoration with high-quality composite
Monitor for symptom resolution
Anatomy was done perfect but patient complained that restoration was high so I decreased the anatomy + ( previous treatment same tooth make it hard to make accurate anatomy , but the most important for patient is function 🦷✅😁👌🏻

🧠 Lesson: Sometimes, conservative treatment is the right treatment. Trust the pulp — it can heal if we let it!

A 20-year-old systemically healthy patient with a smoking habit presented with lingering pain to cold, heat, and sweets, especially at night. Pain intensified over days.

🔍 𝗖𝗹𝗶𝗻𝗶𝗰𝗮𝗹 𝗘𝘅𝗮𝗺 :
Deep caries on lower right molar. Positive cold & heat test with lingering pain. No mobility or swelling.

🧠 𝗗𝗶𝗮𝗴𝗻𝗼𝘀𝗶𝘀 :

Pulp: Symptomatic Irreversible Pulpitis

Periapical: Symptomatic Apical Periodontitis with External Root Resorption

✅ 𝗧𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁 :
Non-surgical RCT to remove infection, stop resorption, and save the tooth.

🔧 𝗣𝗿𝗼𝗰𝗲𝗱𝘂𝗿𝗲 :
Rubber dam, WL determination, rotary cleaning, NaOCl + EDTA irrigation, obturation with gutta-percha + resin sealer, and composite final seal.

📸 𝗣𝗼𝘀𝘁-𝗼𝗽 :
Dense fill to WL, proper apical stop, good coronal seal.
🩹 Prognosis: Favorable — follow-up planned every 3 months, full evaluation at 6 months.
🔁 𝗙𝗼𝗹𝗹𝗼𝘄-𝘂𝗽 𝗣𝗹𝗮𝗻 :
Although the formal healing outcome will be assessed at 6 months, the patient will be monitored every 3 months to ensure proper progression of healing and absence of symptoms.

3-month follow-up: Clinical exam + radiograph to confirm reduction in symptoms and signs of periapical healing.

Clinical and radiographic evaluation performed to monitor the healing progress of periapical tissues and assess the status of external resorption. Continued observation is recommended; full evaluation and confirmation of healing or stabilization are planned for the 6-month follow-up.
