Skipping Breakfast May Increase Stroke Risk

Eating breakfast every day may be beneficial for the prevention of stroke, particularly cerebral hemorrhage, a new study suggests.

The study, published in Stroke on January 5, was conducted by a team led by Yasuhiko Kubota, MD, Osaka University Graduate School of Medicine, Japan.

They note that several previous reports from both Western and Asian countries have shown missing breakfast has been associated with a higher prevalence of obesity, hypertension, dyslipidemia, and glucose intolerance, which are well-known risk factors of cardiovascular disease.

However, no guidelines have yet recommended that adults eat breakfast every day to prevent cardiovascular disease because there is not enough evidence to support such a recommendation.

They therefore conducted the current study to prospectively investigate whether skipping breakfast is related to increased risks for stroke and coronary heart disease in the general Japanese population.

"To the best of our knowledge, this is the first study to investigate the associations of breakfast intake frequency with cardiovascular events that include not only CHD but also stroke and its subtypes in an Asian population," they write.

For the study, 82,772 Japanese men and women aged 45 to 74 years without a history of cardiovascular disease or cancer were followed from 1995 to 2010. Participants were asked about their breakfast eating habits at baseline and classified as having breakfast 0 to 2, 3 to 4, 5 to 6, or 7 times per week.

During the 15-year follow-up, there were a total of 3772 strokes (1051 cerebral hemorrhages, 417 subarachnoid hemorrhages, and 2286 cerebral infarctions) and 870 cases of coronary heart disease.

Multivariable analysis showed that participants never eating breakfast had higher risks for total cardiovascular disease and total stroke, as well as a particularly higher risk for cerebral hemorrhage, compared with those consuming breakfast every day.

No significant association between the frequency of breakfast intake and the risk for coronary heart disease was observed.

Similar results were observed even after exclusion of early cardiovascular events.

Noting that the major risk factor for cerebral hemorrhage is hypertension — especially an increase in blood pressure in the morning — the researchers point out that in the current study those having breakfast every day were less likely to have high blood pressure than those skipping breakfast, although this was not significant.

They also report that several previous studies have shown that cerebral hemorrhage occurs more frequently in the morning than during the remainder of the day, and other studies have reported reductions in blood pressure from eating breakfast. One group has suggested that skipping breakfast was associated with overactivity in the hypothalamic-pituitary-adrenal axis because of a longer period of fasting, leading to elevated blood pressure in the morning.

"This effect of breakfast intake on blood pressure in the morning might be one of the mechanisms of the result from the present study," they speculate.

They note that a previous study has shown a significant association between the frequency of breakfast and the risk for coronary heart disease in US men. They suggest that the difference between this result and the current study may have resulted from the different populations; for example, in Japan the prevalence of coronary heart disease is lower and the prevalence of stroke, particularly cerebral hemorrhage, is higher compared with Western countries.

The researchers point out that they only breakfast habits only at baseline, and it is possible that these may have changed over follow-up. In addition, breakfast skippers may be more likely to have other unhealthy lifestyle behaviors.

The authors state that this study still showed a significant inverse association between the frequency of breakfast intake and the risk for cerebral hemorrhage even after adjustment for various kinds of diet and lifestyle factors, but because it was an observational study there may be other confounding factors that were not measured.

URINARY TRACT INFECTION (UTI)

This refers to any bacterial infection of the urinary tract. Congenital abnormalities of the genito-urinary tract predispose children to UTI.
These infections are much more common in girls and women than in boys and men younger than 50 years of age. The reason for this is not well understood, but anatomic differences between the genders
(a shorter urethra in women) might be partially responsible.
About 40% of women and 12% of men have a urinary tract infection at some time in their life.

CAUSES
• Ascending infection by organisms of the gut flora following bacteraemia or septicaemia

• Urinary obstruction e.g. enlarged prostate in adult males, posterior urethral valves in infants/children

SYMPTOMS

• Frequent painful urination
• Haematuria
• Cloudy/foul smelling urine
• Vomiting
• Suprapubic pain
• Fever
• In children,
• Fever may be persistent and unexplained
• There may be feeding problems, diarrhoea, and failure to thrive as well.

SIGNS

• Fever
• Loin tenderness
• Suprapubic tenderness
• Foul smelling urine

INVESTIGATIONS

• FBC
• Mid-stream specimen of urine for microscopy, culture and sensitivity (re-culture urine after treatment)
• Abdominal ultrasound

TREATMENT

Non-pharmacological treatment/ Prevention

• Liberal oral fluids to encourage good urinary output

• Personal hygiene and proper cleaning after defeacation. Women and girls should wipe from front to back (not back to front) after bowel movements. This helps prevent bacteria from the a**s entering the urethra.

Empty the bladder regularly and completely, especially after sexual in*******se.

Drink plenty of fluids. Cranberry juice, especially, has been shown to help prevent urinary tract infections. There is evidence that cranberries reduce the risk of the bacteria's adhesion to bladder cells.

Alcoholism is common, serious, and expensive. Physicians encounter alcohol-related cirrhosis, cardiomyopathy, pancreatitis, and gastrointestinal bleeding, as well as intoxication and alcohol addiction, on a daily basis. Alcoholism is also associated with many cancers.Wernicke encephalopathy and Korsakoff psychosis are also important causes of chronic disability as well as dementia. Fetal alcohol syndrome is a leading cause of mental retardation. In addition, accidents (especially automobile), depression, dementia, su***de, and homicide are important consequences of alcoholism.

SIGN AND SYMPTOMS

The following are signs and symptoms of alcohol withdrawal:
1. Nausea and vomiting
2. Diaphoresis
3. Agitation and anxiety
4. Headache
5. Tremor
6. Seizures
Visual and auditory hallucinations: Many patients who are not disoriented—and who therefore do not have delirium tremens—have hallucinations

The following are signs of delirium tremens

COMPLICATIONS OF ALCOHOLISM

1. Wernicke encephalopathy: Ataxia, ophthalmoplegia , and confusion.

2. Korsakoff syndrome: Anterograde and retrograde amnesia, often with confabulation and preceded by Wernicke encephalopathy

3. Hepatic encephalopathy: Asterixis and confusion.

MANAGEMENT OF ALCOHOLISM

1. Complete abstinence is the only treatment for alcohol dependence.

2. Emphasize that the most common error is underestimating the amount of help needed to stop drinking.

3. Hospitalize patients if they have a history of delirium tremens or if they have significant comorbidity.

4. Consider inpatient treatment if the patient has poor social support, significant psychiatric problems, or a history of relapse after treatment.

5. Strongly recommend Alcoholics Anonymous (AA)

6. Encourage hospitalized patients to call AA from the hospital; AA will send someone to talk to them if the patient makes the contact

7. Patients need to attend AA meetings regularly (daily at first) and for a sufficient length of time (usually 2 years or more) because recovery is a difficult and lengthy process.

8. In the beginning of treatment, and perhaps ongoing, patients should remove alcohol from their homes and avoid bars and other establishments where strong pressures to drink may hinder abstinence.

9. If the patient has an antisocial personality ( severe problems with family, peers, school, and police before age 15 years and before the onset of alcohol problems), recovery is less likely.

10. If the patient has primary depression, anxiety disorder, or another potentially contributory disorder (the other disorder must antedate the problems with alcohol or it must be a significant problem during long periods of sobriety), treat this primary problem aggressively

PROGNOSIS

The prognosis for alcoholism should not be considered hopeless. As many as 30% of persons with alcoholism stop drinking. Even a patient with cirrhosis might have a favorable prognosis if alcohol cessation is achieved.

Treatment

It depends on several factors. Take into consideration the patient’s age, health, and medical history as well as the type of fracture, its severity, and any resulting brain injuries.

In some cases, such as in basal skull fractures, medication to control pain may be all that is needed. The skull will heal itself in these instances.

However, a basal fracture may require surgery if it results in excessive leakage of cerebrospinal fluid from the nose and ears.

Surgery is more often a required course of treatment for depressed skull fractures. If the
depression is severe enough, surgery may be needed to correct it.

Surgery may also be performed
if the depression is putting pressure on the brain or if there is cerebrospinal fluid leakage.
