New and emerging technologies related to breast cancer -Screening

S.3 Name and purpose: Optical Breast Imaging (OBI) for breast cancer screening

Administration: Imaging

Mechanism: OBI measures the absorption and scattering of near-infrared light through the breast tissue. Cancerous tissues have different light absorption and scattering properties than healthy tissues due to changes in blood volume, oxygen saturation, and water content [8]. OBI can detect changes in tissue density, blood flow, and metabolism that suggest cancer growth.

Strengths and Weaknesses: OBI has several advantages over other breast imaging techniques, such as mammography, ultrasound, and MRI. It is ionizing radiation-free, less expensive, and can detect small tumors in dense breasts. However, OBI has lower sensitivity and specificity than other methods and cannot replace biopsy for a definitive diagnosis.

R&D status: OBI is not yet widely available for clinical use but is currently under-going clinical trials and further development. The US FDA has approved some OBI devices for investigational use in clinical trials.

New and emerging technologies related to breast cancer -Screening

S.2 Name and purpose: liquid biopsy for breast cancer screening

Administration: blood test

Mechanism: liquid biopsy involves the analysis of cancer biomarkers that are shed from tumors into the bloodstream. CTCs, DNA, and proteins are isolated from the blood sample and then analyzed using various techniques to determine their presence and quantity [7]. This method can also be used to monitor the effectiveness of cancer treatments.

Strengths and Weaknesses: The strengths of liquid biopsy compared to other methods include the minimally invasive nature of the procedure, its ability to detect early-stage cancer, and the ability to monitor response to treatment. However, it is more expensive and not as widely available as other cancer screening methods like mammography or colonoscopy.

R&D Status: Liquid biopsies are not yet approved as a routine screening method for cancer but are undergoing significant research and development. This technology is currently used in clinical trials and has shown promising results for the early detection and monitoring of several types of cancer, including breast cancer. However, regulatory approval is still needed before it can be widely implemented in clinical practice.

New and emerging technologies related to breast cancer -Screening

S.1 Name and purpose: Abbreviated breast MRI (fast breast MRI) for breast cancer
screening

Administration: Imaging

Mechanism: MRI uses radio waves and a strong magnetic field to produce detailed images of breast tissue. Unlike mammography, MRI does not involve ionizing radiation exposure. The abbreviated breast MRI focuses on the breast tissue and is performed without the use of a contrast agent, which can reduce the overall time and cost of the exam. MRI is particularly useful in detecting breast cancers in dense breast tissue and in younger women.

Strengths and Weaknesses: Abbreviated breast MRI is more sensitive than mammography in detecting breast cancer in women with dense breast tissue or at high risk for breast cancer [5]. However, MRI is more costly and time-consuming than mammography and may lead to more false positives. MRI is also not suitable for women with certain types of implants or pacemakers.

Regulatory approval: The abbreviated breast MRI was approved by the FDA in 2018. Its use has increased in recent years as a screening tool for women with dense breast tissue or at high risk for breast cancer, but insurance coverage for this exam varies by state and insurance provider [6]. Ongoing research is exploring ways to optimize the exam for certain populations and to better understand its overall impact on breast cancer screening and diagnosis.

P.3 Name and purpose: breast cancer vaccines for prevention

Administration: through injection

Mechanism: The vaccines are designed to target specific cancer-causing proteins such as HER2 and MUC1, which are often found in high levels in the breast tissue of women with high susceptibility to breast cancer [4]. The vaccines stimulate the immune system to target these cancer-causing proteins, preventing their abnormal growth and spread.

Strengths and Weaknesses: This method of prevention has the potential to be more effective than traditional prevention methods, such as mastectomies, as it targets the underlying causes of breast cancer. However, it may not be effective for all types of breast cancer and could require multiple doses over an extended period.

R&D Status: The vaccines are still in the early stages of development and have not yet received regulatory approval. Clinical trials are currently underway to test their safety and efficacy, though it may be several years before they are widely available to the public.

New and emerging technologies related to breast cancer
-Prevention

P.2 Name and purpose: Gene therapy for breast cancer prevention

Administration: The therapy can be delivered directly through viral vectors or
nanoparticles into the cells or tissues.

Mechanism: Different methods are being developed for gene therapy, one is by replacing the mutated BRCA1 and BRCA2 genes with healthy copies, and another approach is to use CRISPR/Cas9 to delete mutations or allow for the insertion of healthy copies [3]. These modified genes are delivered into breast cells using viral vectors or nanoparticles. Once they enter the cells, the genes replace or repair the mutated genes, thereby reducing the risk of the cells developing into cancer.

Strengths and Weaknesses: The strength of gene therapy is that it targets the root cause of breast cancer, giving the potential for long-term protection against the development of breast cancer. Furthermore, gene therapy has a lower risk of side effects than traditional treatments. However, its weakness lies in its significant cost and the delivery method used, which can raise safety concerns.

R&D Status: Gene therapy for breast cancer prevention is still in the preclinical stages of research, and clinical trials are yet to begin. Therefore, it is not yet approved by regulatory bodies.

Maria's life took a remarkable turn when she discovered the life-saving potential of a mobile screening unit at her workplace. 🏢 Being able to access it played a vital role in her survival. Her journey demonstrates how improvements in screening technology can make a profound difference. 🙌💪
Here is the story of her diagnosis:

“In May of 2019, I got my annual mammogram at the mobile van, and the very next day I received a call saying that they needed to take more pictures. Because I was already a Fox Chase patient, they were able to schedule me within days for follow-up screening.
Seeing that Fox Chase cared enough to bring their facility to me via the mobile unit to make sure I got the screening I needed, why would I go anywhere else? After a diagnostic mammogram and subsequent biopsies, I was diagnosed with estrogen-positive breast cancer in my breast and lymph nodes.
I was shocked. None of my family members had a history of breast cancer. I didn’t smoke, and I had successfully breastfed all three of my children, both of which should have reduced my risk. In fact, I’d just run a half marathon not long before getting that mammogram! Breast cancer was just not something that had been on my radar at all.”
