Very often patients come to us with a problem that is interfering with activities that are important to their lives.

Sometimes, we may need to advise them to stop such activities for optimal recovery, and that could be the worst news to them. However, to make it suck a little less, give your patient a definite timeframe for review.

Try saying ‘We need you to stop playing sports for 2 weeks and see if your body is ready to be back after that period of rest, then we’ll see if you need more rest or some modifications for your training’ instead of ‘You need to stop playing sports for now’.

This informs our patients about the treatment plan and give them reassurance that we want the same thing - which is getting them back to the things they love or have to do.

大家對肌腱二字未必熟悉,卻可能聽聞過跑者足跟痛(阿基理斯腱炎)、瑜珈臀部痛(膕繩肌腱炎)、辦公室人員網球肘及哥爾夫球肘(肱骨外側/內側上髁炎)、及球類運動員肩膊痛(肱二頭肌/肩袖肌群肌腱炎),以上種種痛症都是肌腱問題。

肌腱問題的處理重點是休息減壓及逐步重新適應壓力以改善肌腱健康。物理治療師可以不同治療方法如超聲波、針刺、電療等減輕炎症及放鬆相關肌肉,再以根據病人的訓練或生活習慣給予意見,及制定復康運動計劃逐步訓練肌腱強度,預防痛症復發。

You may not have heard of the word ‘tendon’, but you may be familiar with these: heel cord pain in runners (Achilles tendinopathy), buttock pain in yogis (hamstring tendinopathy), tennis or golfer’s elbow in office warriors (lateral/medial epicondylitis) and shoulder pain from biceps or rotator cuff tendon in overhead athletes are all examples of tendinopathy.

A period of unloading and gradual reloading is required to manage tendinopathy and optimise tendon health. Physiotherapists can utilize different treatment modalities like ultrasound, needling and electrotherapy to relax the relevant muscles and decrease inflammation to promote unloading and recovery and give advice on training or daily life modification and a rehabilitation exercise program for gradual reloading of the tendon with an aim to improve its resilience to loading and prevent recurrence of pain.

為病人檢查及治療時談天非常重要。

閒談間了解他人的生活方式、嗜好、工作等比傳統的問診感覺輕鬆,談到天南地北時亦可回到正題給予生活習慣的意見,一舉兩得。

睡眠充足、多飲水、做運動都是老生常談,而我每次都會跟病人說:「其實大家都知道怎樣生活才健康,問題是你有沒有將自己的健康放在首位?當你把健康放在首位,再計劃生活,生活就會因為許多小改變而變得不一樣。」

知易行難,共勉之。

Chitchatting with patients during assessment and treatment is very important.

Casual talking makes it a lot easier for patients to talk at ease about their lifestyle, hobbies and work then traditional subjective examination, and giving advice during interesting chats makes your advice more tailored to the patient's needs and lifestyle.

Speaking of lifestyle, we all know that we need to sleep, drink water, exercise blah blah blah, on this topic, my advice is usually this: we all know what is healthy lifestyle, the true problem is we are not putting our health as our priority. If we put our health first then plan our lives around it, the small changes we make will change our lives.

It's easier said than done, let's make it happen!

Thank you once again for the invite from to host this rigid taping workshop! It's been two fun days sharing more about working in sports setting and rigid taping in both on-field and clinic setting.

Stay tuned for some upcoming workshops/seminars! Working as a physio is hard (on your fingers especially for rigid taping) but it surely is one of the most rewarding job one can ever have :)

當肌肉發力過大超過極限,肌肉就會拉傷。肌肉拉傷分為三級:第一級是輕微拉傷,第二級是部份撕裂,第三級是完全撕裂。
⠀⠀⠀⠀⠀⠀⠀⠀⠀
一級拉傷通常需要簡單的護理及適度休息達致復原。主要癥狀有輕微腫脹、受傷位置疼痛、沒有瘀傷及良好的活動幅度。傷勢應在傷後第一星期內有明顯改善,以及在2至4星期內完全康復。
⠀⠀⠀⠀⠀⠀⠀⠀⠀
若你的疼痛持續兩星期以上,或跟以上所述有別,就可能是二或三級拉傷,需要進行物理治療以舒緩疼痛及回復關節功能。二及三級拉傷部位會有中度或嚴重疼痛,痛感或會擴散至周邊;亦會有瘀傷、明顯腫脹、關節活動幅度受限等癥狀。患者的康復時間需根據傷勢判斷,短則8星期,長則6個月或以上。

Physiotherapists can treat muscle strain injury, be it pain in the biceps after lifting a heavy package, or pain in the lower back when picking your baby up.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
Muscle strain injury happens when force that exceeds the capability of the muscle is exerted, causing an injury. It is classified in 3 grades: Grade 1 for minor injury, Grade 2 for partial tear and Grade 3 for complete tear.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
Grade 1 injury requires standard care and relative rest to facilitate recovery. It has minimal swelling, slight to mild pain on the injury site, no bruising, good range of motion, and it should show good progress of recovery within a week, and full resolution within 2-4 weeks.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
If your injury bothers you for more than 2 weeks and is not behaving like above, it is likely to be a Grade 2 or 3 injury. such injury will require treatment from physiotherapists for pain resolution and restoration of function, or even surgery. These injuries present with significant pain on and around the injury site, swelling, bruising and limited range of motion. The recovery timeframe can range from 8 weeks to 6 months or above depending on severity.

If you are injured, get your physio to offer appropriate assessments and treatments to prevent other problems down the road!

許多長期痛症都因代償問題而牽涉脊椎,所以核心訓練常常成為治療計劃中重要一環。我的病人聽到核心訓練就會問我:「我應該怎樣練腹肌?」

練核心不只練腹肌。核心肌群由四組肌肉組成,包括腹肌、背肌、橫隔膜(負責呼吸的肌肉)及會陰肌。這四組肌肉一起助你保持腹壓,加強脊椎穩定性及運動能力。
⠀⠀⠀⠀⠀⠀⠀⠀⠀
核心訓練不只有仰臥起坐,抬腿及平板支撐,亦不應該以盡力收緊核心的方式進行運動。收緊核心的動作會令腹壓過大,影響會陰肌健康,有機會導致失禁。進行抬舉時盡力收緊核心也是腰背痛病人的活動習慣,有機會是痛症引致的代償。訓練核心肌群應以流暢的肢體活動令四組肌肉輕鬆同時有效地控制腹壓。臨床普拉提以配合病人痛症方向性的方法進行運動,是訓練核心肌群的好方法。

A lot of chronic pain problems have a central component due to compensation strategies which often happen in the spine. Hence, core training is very often a vital part of the treatment plan. When I talk about training core, a lot of my patients will ask, ‘then how do I train my abs?’
⠀⠀⠀⠀⠀⠀⠀⠀⠀
Training your core doesn’t mean training your abs alone. Your core is formed by 4 muscles groups, abdominal muscles, back muscles, diaphragm (the breathing muscles) and pelvic floor (the muscles in the saddle area). Together, they all work to maintain intra-abdominal pressure for spinal stability and optimal motor function.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
Core training doesn’t only mean sit-ups, leg raises and planks, and it shouldn’t be done in a way where you need to ‘brace your core’. The act of bracing will build up excessive intra-abdominal pressure and is detrimental to your pelvic floor health which may lead to incontinence. It is also the movement pattern that patients with low back pain adapt for lifting tasks, which may be a compensation for their pain. What we should look for is training with smooth movements to encourage effective control of intra-abdominal pressure with ease by the 4 muscle groups in the core. Clinical Pilates is a great way to achieve that with directionally-matched exercise.

治療師可透過這類手法快速活動關節,減低周邊肌肉張力,從而增加活動幅度及減輕痛楚。有時候聽到的咔啦聲是關節裏面的氣泡因氣壓改變而爆破的聲音,對治療效果沒有影響。所以就算沒有聲音,這類手法都會對痛症有幫助!

最重要的是在進行手法前和後重覆檢查,了解手法是否有效。

Hearing cracks from manipulation makes a lot of people feel relaxed, but it also makes the myth of 'manipulation is not successful without crack sounds' popular these days. (Still!)

Manipulation is a manual therapy where a therapist moves a joint through range quickly to lower muscle tone surrounding the area in order to improve quality and quantity of movement and reduce pain. The cracks that we hear sometimes is a result of air bubbles bursting in joints because of air pressure change caused by the technique, but it has no relationship with the treatment effect. In other words, a technique could be effective without the crack!

The most important thing about this kind of therapy is to perform examination before and after to know if it's effective or not.

許多都市痛症的成因跟生活習慣有關,久坐、缺乏運動、睡眠不足等令肌肉緊張及關節僵硬造成痛楚。物理治療能有效舒緩痛症,但治本之道是改善生活習慣,保持身體健康。若病人接受物理治療後一切如舊,痛症就會復發,繼續影響生活。

受傷後因受傷部位功能變差誘發的痛症亦一樣,我們能將症狀減低,但治本之道是鍛練患側,令身體回復平衡,再保持運動習慣以防功能衰退。若病人在痛症痊癒後不再運動亦會令痛症復發。

就如房間一樣,不打掃就會積累灰塵,要保持清潔就要常打掃。清潔如是,健康如是。

One of the most common questions asked during consultation is: “Will my pain be gone once and for all?”
⠀⠀⠀⠀⠀⠀⠀⠀⠀
Many common pain problems are closely-knitted with lifestyle. Factors like prolonged sitting, being sedentary, lack of sleep could all make us prone to having tight muscles and stiff joints, causing pain. Physiotherapy is great for managing the symptoms, but the ultimate cure is lifestyle changes and being healthy. If patients just go on with their lives without making changes, the pain will creep back into their lives.

The same goes with pain that results from an old injury that worsens function of the particular body part. We can manage the symptoms, but to cure the problem we need to train up the problematic side to regain balance, then keep exercising to maintain the balance and function. If patients stop exercising after the problem is gone, it may come back and haunt them again.

If we don’t clean our rooms often, dusts accumulates and the room gets dirty. If we wish to keep our room clean, the only way to do it is to clean constantly. Same goes for health. We have to work on it constantly to keep ourselves healthy.

物理治療是淋巴水腫的主要治療方法。淋巴水腫分為原發性(遺傳性,較不常見)及繼發性,即由手術、放射治療或腫瘤引起的水腫。

淋巴水腫在乳癌病人之間十分常見,約每三位乳癌病人就會有一位患上淋巴水腫。常見病癥包括:患肢脹大及感到沉重、不適甚至疼痛、發現衣物及配飾比以前繃緊、皮膚繃緊及乾燥、及患肢靈活度下降等。

物理治療師可以淋巴引流手法、壓力繃帶、漸進壓力電泵及運動治療淋巴水腫。我們亦會指導病人作自我引流以減低日常不適,及提醒一些日常生活的注意事項。

Physiotherapists play a big role in lymphedema management. Lymphedema could be primary or secondary to surgery, radiation therapy or space-occupying lesion.
⠀⠀⠀⠀⠀⠀⠀⠀⠀
Lymphedema is a common condition in breast cancer survivors. approximately 1 in 3 breast cancer survivors develops lymphedema in the arm on the affected side. Common signs and symptoms of lymphedema include increase in size and feeling of heaviness, discomfort or pain on the affected limb, feeling of constriction from clothing, jewellery and watch that fit before, tight and dry skin on the affected limb and difficulty in moving the affected limb.

Physiotherapists can help manage lymphedema with manual lymphatic drainage technique, compression bandaging, sequential pump and gentle exercises. We also teach patients with lymphedema to perform self-drainage techniques to manage their symptoms at home, and educate them on precautions to take in daily activities.
