【專欄文章】TOPick — 醫語怡情︰幽門螺旋菌會傳染嗎？

都市人工作壓力大，食無定時，偶有胃氣脹、胃痛等症狀也可能習以為常。但如果長期出現以上不適，尤其與飲食習慣無明顯關聯，便要提高警覺，因有機會與幽門螺旋菌感染有關，長遠亦可能增加患上嚴重胃病的風險。

幽門螺旋菌感染十分常見，有本地大學研究發現，亞洲有超過一半人口感染幽門螺旋菌。幽門螺旋菌是一種可在胃部及十二指腸內生存的細菌，它會分泌尿素酶（Urease）以中和胃酸，但同時刺激胃酸分泌並損害胃壁，從而演變為消化性潰瘍。幽門螺旋菌多經由口水傳播，故保持個人衞生相當重要，與帶菌者同住或共餐、共用牙刷或餐具，以及飲用受污染的水源或進食未徹底煮熟的食物等，都有機會造成感染。

由於大部分患者感染初期多數沒有明顯症狀，故很容易被忽略。當感到胃部不適時，如何分辨是一般胃病或是幽門螺旋菌感染？前者非由細菌感染所致，症狀通常於進餐後出現；但若長期出現消化不良、上腹疼痛等症狀，尤其是空腹或晚上睡覺期間因腹而醒來，甚至排黑便，則有可能是幽門螺旋菌感染，應盡快求醫。檢測幽門螺旋菌可透過尿素吹氣檢測、糞便抗原檢測及血清抗體檢測等非入侵性檢查；醫生會因應患者情況建議接受胃鏡作深入檢查。尿素吹氣檢測是現行最普遍的測試方式，但對於40歲以上、曾患胃潰瘍或家族有胃癌病史的人士，則建議直接進行胃鏡檢查，以排除其他胃部疾病的可能。

治療方面，現時較常採取「三聯療法」，即以抑制胃酸分泌的藥物（如質子泵抑制劑）配合兩種抗生素，以根除幽門螺旋菌，療程通常為兩星期。只要患者依從指示用藥，療效一般可達80至95%。現時更有新型抑酸藥（鉀離子競爭性酸抑制劑），抑酸效果更強、更穩定，可用以取替質子泵抑制劑，提高根除成功率。醫生一般會於療程後再次安排檢測，以確定細菌已被徹底根除。

不過，近年抗生素抗藥性問題愈見嚴重，部份患者有機會在接受上述療程後仍未能根治。曾有中年女患者因工作壓力大，長期上腹疼痛、胃氣脹及消化不良，起初以為一般胃病，但服用特效胃藥後仍未有改善，其後更排黑便，擔心可能胃出血。經胃鏡檢查確定為幽門螺旋菌感染引致的十二指腸潰瘍。醫生先處方兩星期「三聯療法」，惟療程後幽門螺旋菌檢測仍呈陽性，因此需改用「四聯療法」，即在原有抑酸藥物及兩種抗生素的基礎上，加入鉍劑，然而，此療法出現副作用的機會較高。雖然患者最後能成功根除幽門螺旋菌，但如能及早發現和治療，可避免病情惡化及引致胃癌等嚴重胃病的風險。

治療幽門螺旋菌後，日後仍有機會再次受感染，因此日常生活中應保持良好個人及飲食衛生，例如用餐時使用公筷、避免食用不潔的生水與食物等。同住家人及親密接觸者亦應在患者確診後接受幽門螺旋菌檢測，及早介入治療，避免交叉感染。

🔔接種疫苗　提防侵入性 B 型腦膜炎雙球菌（MenB）

最近英國爆發侵入性 B 型腦膜炎雙球菌（MenB）群組感染，短短一星期內錄得 20 宗確診，包括兩名學生不幸離世，並有重症個案需要入住 ICU。事件發生喺學生聚集嘅肯特郡同倫敦一帶，當地政府已將事件列為全國緊急情況，而香港衞生署衞生防護中心亦與英國跟進中。由於當地亦係本港留學生嘅熱門留學地點，病毒傳播風險不容忽視！

🧠 咩係侵入性 B 型腦膜炎（MenB）？
MenB 屬於一種可快速惡化嘅細菌感染，會攻擊腦膜及脊髓保護層，青少年及生活環境密集嘅學生群組特別容易受感染。傳播途徑包括飛沫、喉嚨分泌物、密切接觸、共用飲品或物品。

⚠️ 出現以下症狀需立即求醫：
• 高燒
• 頸部僵硬
• 頭痛
• 畏光
• 皮膚出疹，按壓不褪色（可用「玻璃杯測試」）

💡 如何保護自己？
✔ 1. 接種 MenB 疫苗（最有效預防方法）
英國爆發感染其中一個重點，就係大部分青少年未有接種 MenB 疫苗，而現行嘅 ACWY 並唔能夠預防 MenB。
✔ 2. 注意衞生，避免高風險接觸
• 避免共用水杯、吸管等個人物品
• 保持良好個人衞生
• 如有不適立即求醫
✔ 3. 及早使用抗生素至關重要
一旦懷疑有感染嘅情況，醫生會立即使用抗生素處理；越早介入，治療越有效。

🔔 Protect against Meningococcal Infection with timely vaccination
A recent cluster of Serogroup B Meningococcal Disease (MenB) cases has been reported in the UK, with 20 confirmed cases recorded within just one week, including the unfortunate deaths of two students and several severe cases requiring ICU care. The outbreak occurred mainly in student‑dense areas such as Kent and London, and local authorities have classified it as a national emergency. Hong Kong’s Centre for Health Protection is closely monitoring the situation with the UK. As the UK remains a popular study destination for Hong Kong students, the potential risk of transmission should not be overlooked.

🧠 What is Serogroup B Meningococcal Disease (MenB)?
MenB is a rapidly progressing bacterial infection that affects the protective membranes surrounding the brain and spinal cord.

Teenagers and young adults living in crowded or high‑density social environments are at higher risk.

MenB is spread through respiratory droplets, throat secretions, close contact, and sharing drinks or personal items.

⚠️ Seek medical attention immediately if these symptoms appear:
• High fever
• Stiff neck
• Headache
• Sensitivity to light
• Skin rash that does not fade under pressure (you can use the Glass Test to check)

💡 How can you protect yourself?
✔ 1. Get the MenB Vaccine – the most effective protection
One key factor behind the recent UK outbreak is that many teenagers had not received the MenB vaccine. Please note that the commonly used ACWY vaccine does NOT protect against MenB.

✔ 2. Maintain good hygiene & avoid high‑risk contact
• Avoid sharing cups, straws, or personal items
• Practise good personal hygiene
• Seek medical care promptly if you feel unwell

✔ 3. Early use of antibiotics is crucial
When MenB infection is suspected, doctors will begin antibiotic treatment immediately. Early treatment significantly improves outcomes.

Stay alert, recognising symptoms early, avoiding high‑risk exposure, and getting the MenB vaccine can greatly reduce your risk.

【專欄文章】TOPick — 醫語怡情︰及早識別警號妥善管理壓力

生活中，無論是瑣碎的行程延誤，還是家庭與工作的重擔，都可能成為壓力來源。適度的壓力能提升專注力和表現，但若長期處於高壓狀態，便可能增加患上心血管疾病等風險。

醫學上雖沒有對「壓力」作出明確定義，但一般可理解為面對挑戰或變化時身體和心理上會出現的自然反應。壓力可分為急性與慢性兩大類。急性壓力持續時間短，來得快走得快。例如與人爭執、趕巴士、因塞車而遲到等；慢性壓力可能持續數週甚至幾年，如財困、家庭糾紛、照顧患病親人、工作壓力等。

面臨壓力時，身體會啓動交感神經系統（sympathetic nervous system），瞬間釋放腎上腺素荷爾蒙（adrenaline），引發「戰鬥或逃跑」反應（fight or flight response）。短短幾秒鐘心跳加快、呼吸急促，拼命把血液和氧氣送到全身細胞。血糖會上升為身體準備爆發性能量，感官變得敏銳，肌肉繃緊，精神進入高度專注狀態。這種反應是演化而來的生存機制，讓我們能迅速逃離險境或對抗敵人。然而，現代生活中的壓力，也會令部分人頻繁地觸發此反應機制。若長期處於高壓狀態，便會損害身體多個系統。

以心血管系統為例，慢性壓力會令血壓上升，長遠會增加患中風及心臟病風險；若本身為過敏體質人士，可能更容易濕疹或哮喘發作。另外，壓力會影響腸道運作，腸易激綜合症患者可能更難以控制症狀，例如腹痛、腹瀉等。免疫力亦會因壓力而下降，可能更易感冒、「生蛇」等；而皮膚也可能出現問題，例如蕁麻疹或脫髮。除此之外，慢性壓力更會引致食慾增加，使人偏好高糖高脂的高能量食品，進而體重增加、中央肥胖、增加糖尿病風險。心理上，若壓力無法紓緩，容易出現情緒低落、易怒等情況，甚至可能引發焦慮症和抑鬱症。壓力會令人提高警覺性，因而難以入睡，導致失眠。與此同時，睡眠不足亦會加劇壓力，形成惡性循環。

臨床上，若懷疑病人出現壓力相關症狀，醫生會檢查身體、量血壓，甚至安排其他檢查項目，例如心電圖和驗血糖及甲狀腺功能等，以排除其他疾病偽裝成壓力。醫生亦會評估病人心理狀態，評估有否焦慮症和抑鬱症的風險。曾有一位中年女士因血壓高前來求診，情況已持續約三個月，但她本身沒有高血壓病史。深入了解及檢查後發現，她有一名獨居親人因中風入院，她每日需兼顧探病、家庭與工作，感到壓力沉重，導致失眠與情緒不穩，繼而發現血壓上升。後來家人分擔照顧患病家屬，她的睡眠得到改善，壓力也減輕了，其血壓亦恢復穩定。

壓力也許無法避免，故懂得管理壓力對身體健康非常重要。定期運動能促進「安多酚」分泌，這種荷爾蒙有助減壓、紓緩焦慮情緒，令人運動後感覺身心舒暢。每週目標為150分鐘的中等強度運動（例如急步行）；沒有運動習慣的人，可以由低強度運動開始，循序漸進。選擇自己喜歡的運動，或相約朋友、伴侶一起進行，會更容易持之以恆。瑜伽結合伸展與深慢呼吸，有助放鬆繃緊的肌肉；太極則透過流暢緩慢的動作，讓人專注當下，平靜心神。此外，可以定期投入一些能為自己帶來滿足感或喜悅的興趣。

良好的睡眠習慣亦會改善睡眠質素，幫助減輕壓力。成年人每日理想睡眠為七至九小時。營造良好睡眠習慣，如固定時間睡覺起床、睡前避免看手機等藍光電子產品、保持睡房溫度舒適、調暗燈光、播放輕柔音樂等均有助入睡。保持均衡飲食，遠離煙酒，並避免高糖加工食品，有助減少血糖大幅波動，從而穩定情緒。下次飯前感到「口痕」，可以嘗試進食少量原味無添加鹽的果仁。孤單或會加重壓力感，應保持定期的社交活動。甚至可以尋求專業幫助，由臨床心理學家進行心理輔導，學習壓力管理的技巧。如果壓力嚴重影響日常生活，可能需要精神科專科醫生介入，配合藥物與心理治療，以回復身心平衡。
