【專欄文章】TOPick — 醫語怡情︰微創冷凍消融術治療肺癌

肺癌是香港頭號癌症殺手，2023年錄得超過六千宗新症。傳統治療以外科手術切除腫瘤為主，再視乎情況輔以化療、標靶治療、免疫治療等。不過，手術對部分病人而言，創傷較大，恢復期較長。近年引入以冷凍消融術治療肺癌，可為不適合接受外科手術的病人提供多一個微創治療選擇。

冷凍消融術在本港以往多用於治療腎癌，近年開始應用於肺癌。一般而言，若肺腫瘤屬早期，且尚未擴散至淋巴或其他器官，經醫生評估合適的病人可考慮接受冷凍消融術，以根治為目標；此療法以微創方式進行，因此適合心肺功能較差、身體狀況不宜接受手術的病人。對於癌症擴散至肺部的病人，即使正接受標靶治療或化療等舒緩治療，亦可在毋需停藥的情況下接受冷凍消融術。由於術後可減少止痛藥劑量，更有助提升病人的整體生活質素。

冷凍消融術，顧名思義，就是利用極低溫將腫瘤組織冷凍並破壞，從而達致治療效果的一種微創介入治療方法。病人在接受麻醉後，介入放射科醫生會利用一支直徑僅約三毫米的冷凍探針，在電腦斷層掃描的引導下，精準鎖定肺腫瘤位置，經病人皮膚直接穿刺至腫瘤。探針在刺入腫瘤後會急劇降溫至零下140度，令癌細胞中心凝結成冰晶，從而破壞其結構及運作；治療過程會進行冷凍及回溫的循環，而腫瘤邊緣位置會維持在0度至零下40度之間，癌細胞組織內會形成冰球，由於癌細胞內外的溫度反覆急速轉變，滲透壓差異會令細胞死亡。同時，腫瘤附近的血管也會因急劇冷凍而收縮，促使癌細胞因缺血而壞死。

整個療程約需一至一個半小時，病人在術後一般只需留院觀察一晚，部分病人可能會出現傷口發炎、低燒、作嘔等輕微副作用，但大多在數天內復原。病人其後需要定期接受電腦掃描檢查，以確保癌細胞已被徹底消滅。

有醫學文獻指出冷凍消融術成效理想，逾九成可完全消滅腫瘤。曾有六十多歲女士因體檢而發現原發性肺腫瘤，在接受冷凍消融術後，翌日已可出院，跟進檢查顯示腫瘤持續縮小，毋需作進一步治療。冷凍消融術相比切除手術的創傷性較低，可保存較多肺功能，並不會影響周邊健康組織。此療法尤其適用於多發性肺腫瘤或復發轉移肺癌的病人，醫生可因應病人的病情及治療需要，安排病人重複接受療程。外國更曾有個案累計接受超過三十次冷凍消融術。

近年本港有醫院引入冷凍消融術治療肺癌。隨著醫療科技日益進步，肺癌的治療選擇越來越多，但由於每位病人的情況不同，建議病人與醫生商討最合適的治療方案。

【Study findings show that shingles is closely linked to heart health】

Shingles (herpes zoster) is not just about skin rashes and nerve pain, but may also pose potential risks to cardiovascular health. 【疫苗迷思：自然感染雖然「天然」，但真的是更理想的保護方式嗎？】
「與其接種疫苗，不如親身感染一次，自然就會有抗體？」這個想法聽起來好像順其自然，但往往忽略了感染過程本身可能帶來的風險，而這些風險，正是疫苗能夠幫助我們避免的。

不少傳染病在感染過程中，可能引致肺炎、腦炎等嚴重併發症，甚至留下長遠健康影響。以麻疹、小兒麻痺症或破傷風為例，這些疾病在疫苗普及之前，曾對無數兒童的健康以至生命構成威脅。對於免疫系統仍在發育階段的兒童而言，感染後出現重症或後遺症的風險尤其值得關注。

疫苗並非要取代人體免疫系統，而是在無需經歷患病過程和承受疾病風險的前提下，協助身體建立免疫保護。世界衞生組織資料顯示，過去50年間，疫苗至少拯救了超過1.5億條生命，並有效預防30多種可致命或引發嚴重後果的疾病與感染。當社區內有足夠多人接種疫苗，疾病的傳播亦會隨之減慢，保護的不只是個人，而是身邊每一個人。

【Vaccine myth: Is natural infection better than vaccination?】
You may have heard that so-called “natural immunity” gained through infection offers better protection than vaccines. What’s frequently overlooked, however, is the real risk posed by the infection itself.

Many infectious diseases can lead to serious complications such as pneumonia or encephalitis, and may result in long‑term health effects. Diseases such as measles, polio and tetanus once posed serious threats to the health, and even the lives, of countless children before vaccines became widely available. Children are particularly vulnerable, as their immune systems are still developing.

Vaccines do not replace the immune system. Instead, they help the body build protection without having to go through the illness and its associated risks. #港怡醫匯 診所現參與政府「大腸癌篩查計劃」，為合資格人士提供大便隱血測試，測試費用由政府全數資助。若測試結果呈陽性，我哋會轉介參加者接受大腸鏡檢查，搵出大便驗出隱血嘅原因，並喺有需要時即時處理瘜肉，減低日後演變成癌症嘅風險。

咩類型人士係合資格參加者？
🔹 50 至 75 歲（即 1950至1976年出生）
🔹 香港居民，持有效香港身份證或豁免登記證明書
🔹 已登記加入「醫健通」（電子健康紀錄互通系統）
🔹 冇大腸癌症狀，亦冇高風險因素如家族病史、曾有大腸瘜肉等
🔹 喺指定時限內，未曾接受相關大腸癌檢查或篩查
📌 須經醫生評估參加者是否適合參與計劃
