YOUR LIFE PLAN - WHERE IS THE SPACE FOR A FAMILY?

The age of first birth is continuously advancing in the western world. Women are postponing pregnancy for social or career reasons. Their partners are not always committed to a family or many times, work demands are taking priority leaving couples with limited time to dedicate to a third person. And today's couples are under the false impression that a pregnancy can always be achieved by the miraculous means of Assisted Reproduction. This is simply not the case.

How would your Life Plan look like if a good understanding of the reproductive reality could be delivered at the beginning of your career? Would the place of the family box shift to an earlier position in your life? Let's face it, in young adulthood it is all about contraception. Would the fact that a woman has no more than 400 days in her life to become pregnant change priorities? Or that fertility starts declining from age 32, sharply from age 35 and the likelihood to conceive per month is as low as 10% after 35years of age?

WHY TELEMEDICINE CAN REVOLUTIONISE YOUR ACCESS TO CARE

In 2020, everything is blamed on the COVID. It is so easy to find a culprit for the shortcomings of one's offering.

Telemedicine, or the ability to see a physician without sharing the same room, has been around for a long time. Yet, many reasons have prevented its full implementation, the list including no possibility to do a full examination and assessment in an acute situation, no blood tests or ultrasound can be done and the lack of support from the medical and insurance community.

Yes, there are concerns about online security and privacy. And indeed the internet is not a foolproof mode of communication. It is the best we have now. Reassuringly, modern platforms like Webex, used by governmental bodies and international organisations have an end to end encryption platform and have built privacy at their core.

At the global level, the benefits of telemedicine far outweigh the risks. Just consider the following arguments.

In the circumstances of non-acute medical needs, telemedicine is an ideal tool to offer all patients the opportunity to meet a doctor without having to travel to her/his office. Modern communication means have the ability to place doctors and patients in contact via full video and audio controllable channels, a true face-to-face consultation. Say Goodbye to travel for any distance to see your doctor, worry about finding a parking space, need to top-up the meter. Say Hello to a consultation in time, no need to attend the hospital or clinical environment, no time spent in a waiting room or registering at reception, a clear slot in your calendar and the ability to organise other commitments around it.

The pro and con debate will continue for the foreseeable future. And where is the customer, the many times forgotten patient in this debate? Have patients made their voices and choice heard? When services are coming to a halt, today due to Corona, tomorrow due to lack of staff, money or goodwill, who will ask the question "Why don't we have Telemedicine medical services as mainstream?". And why should we wait until the next crisis starts and not act now by demanding that Telemedicine should be introduced for all to have access, to support patients in need and willing doctors that wish to provide continuity of services for the ones they care for.

Otherwise, medical needs will soon become "non-essential". Is this what we all want?

What a wonderful mystery! There is no computer programme that can create human life. There are no specific secrets which will guarantee a pregnancy. Some try and are never blessed with a child, others don't even try and are shocked to find themselves pregnant.

The truth of the matter is that society is failing in providing a balanced education to our adolescents who are exposed to contraception advice, as they should be, but never told that fertility is a finite life event, particularly for girls. As opposed to males, who make s***m constantly throughout their lives (genetically they are of less quality as male’s age advances), the availability of a good quality egg decreased as the woman’s age advances. When all eggs have been exhausted, the woman reaches the menopause, around age 51. Her ability to reproduce will stop well before this age, while the man’s continues. At female age 35 the likelihood to become pregnant after one month of trying is only 20%. At 40 it is 10%. Even if a woman will become pregnant after 45 years of age, she will have a high pregnancy complication rate and a small likelihood to deliver an own genetic child.

This is why it is crucial to balance the need for contraception when a pregnancy is not desired with the decision of starting a family young, once comfortable with the thought. There is no ideal time to have a baby and delaying until too late in life might prove a regrettable plan.

Should all young girls and boys be told that a girl has only 400 days in her whole life when she could conceive? I believe, yes. It will focus the minds of both genders as to the ephemere opportunity to have a child. It might strengthen the desire to create solid and long lasting relationships based on good understanding of life and commitment to have a family. It will help balance education, life ambitions, career plans with the realisation that human relationships and taking time to understand each other and why not, have a family, are equally important.
