BIORAD knee failed miserably at just 12 years!

52 yrs female
RA
bilateral TKR was done using BIORAD ORTHOVASIVE ( Indian) implants 13 yrs back.

Presented to us with severe Osteolysis around the tibial component, loosening, varus collapse and a stress fracture of the left proximal tibia.

We have already done revision of her rt knee for similar presentation at just 9 years !

Tibial component just walked out. There was extensive blackish metallosis and synovitis which required extensive synovectomy.
Femoral component was still holding on which required oscillating saw to remove it.

Did semi constrained revision using DePuy TC 3.
Used mobile bearing tibial tray + Uncemented metaphyseal sleeve + Uncemented extension rods on both sides.

PRE COLLAPSE -
Stage II
Inj. Teriparatide sc daily 3 months
Inj. Denusumab sc 1 dose
Inj. Vitanova 6 L ( Vit D) IM 1 dose
Inj. Metadec 50 IM 1 dose

Tablets-
Mext 7.5 F kit (Methotrexate)OD
Lefra (leflunamide) 10 mg OD
Ecosprin Gold 10 OD
Caldon/ CCM/ shelcal 500 mg OD
Fitjoint Max OD

Presidio ( etoricoxib 90+ pregabalin 75) HS x 10 days

Altraday ( aceclofenac 200 + rabeprazole 20 ) OD x 10 days
& sos

Tab / liquid. Uprise D3 60k weekly, 6 months

Idrofos 150 ( ibandronate) monthly 7am with water
6 months

Inj. Denusumab can be replaced with Zoledronic if cost is an issue.

Percutaneous Multiple drilling under c arm without fracture table. (Not core decompression)
Drilling is usually combined with opposite THR which is usually stage 3/4. If combined with opposite THR, drilling is complimentary ( free of cost ).

Core decompression only if patient is willing for and can afford cultured osteoblasts ( ossgrow by Regrow)

STAGE III -
EARLY COLLAPSE
S REGIME + Wait and watch or THR

STAGE IV -
Late collapse and arthritis
UNCEMENTED COC THR
