Ultrasounds are a regular part of prenatal medical care for most pregnant women, and it also provide parents with the first glimpse of their developing baby.
So i wanted to take time and do my part to spread some awareness about ultrasound in pregnancy.

Lot of you guys think that ultrasounds are harmful for your baby and doctor is unnecessarily prescribing it...
So let me start by saying that ultrasounds are absolutely safe in pregnancy ,they use sound waves unlike x-ray etc in which radiations are used.
*

First scan is usually done around 6 weeks to confirm pregnancy,heart beat of baby and to date your pregnancy.
Second scan(level one) is done around 11-13 weeks in which baby's neck thickness and nasal bone is measured( which is important to rule out some major anomalies in baby) along with location of placenta(which if happens to be low lying,you will be advised to take certain precautions).
Next and the most important scan is done at 18-20weeks(Level II) in which detailed study of your baby is done to rule out any abnormality.
After this usually next scan is done around 36weeks to know final baby weight,placenta location etc
And sometimes we recommend a scan around 30weeks (growth scan)as well to assess the baby growth.

These are brief outline,for some patients ,your doctor might run more scans especially if you had bleeding , your baby growth is less,You have high BP or even a fetal ECHO if you are having diabetes in pregnancy.

So at the end,i want you guys to know two things:
Ultrasounds are safe in pregnancy and Level II scan is most important and should not be missed.
So if you know someone living in rural area with sparse medical facilities and awareness, kindly spread this information and advise them to go for scan especially during their 5th month.

So am here again with a very sensitive topic and something that is close to my heart!
Mental health!!
The most important yet most ignored aspect of health.
Don't get me wrong,mental health is equally important for men and women but pardon my partiality towards women because above all human connections..am a women first!
I can go on and on about mental health but let's stick to pregnant mothers for a while...
Pregnancy is just not a women's job,entire family especially husband is a very important part.
But they almost everytime forget their responsibility.They acknowledge all physical symptoms like nausea , vomiting , difficulty to walk but completely forget mental aspect of being pregnant so much so that patients come to us and start crying that there is no one back home who is willing to listen to them.
Husband feels that their job gets over the moment they drop the women to clinic and buy medications for her.
Women are blamed if they are depressed or unhappy and often labelled as 'selfish' for not thinking about the baby. But what you lovely women have to understand is that it's not your mistake and you are not alone.
People expect a woman to be joyful before and after having a baby, and if she is not, she is not a good mother.
What we need is a broad cultural awareness that antepartum and postpartum depression is common.
Pregnancy is challenging enough and having panic attacks or feeling depressed put both you and your baby at risk and trust me getting help early will make things so much better so please speak up,seek help...don't keep things inside.You have to fight for yourself because nobody else will.

And all you lovely family members/friends/husbands reading this post, if you know anyone who is pregnant,please visit them,call them ,talk to them.. visit clinics with them,ask about what they ate,how they feel,make her feel that her happiness means the world to you.
Pregnancy is considered the most beautiful chapter of women's life so let's keep it that way.

Folic acid supplementation during pregnancy is a must!
Infact women should start taking it 2-3 months prior to conception.Experts recommend 400mcg daily before conception and during first three trimester but if your previous child has a neural tube defect or you are on certain medications like anti epileptic etc then the recommendation is 4000mcg daily, throughout pregnancy.

What is folic acid?
It is a form of Vitamin B9 and it plays a crucial role in pregnancy as it prevents certain birth defects (Neural tube defects) like spina bifida affecting spinal cord and anencephaly which affects the brain.Folic acid also helps in red blood cell formation thus help in preventing anemia during pregnancy which is quite common and most important cause of maternal deaths in developing countries.

Why supplementation?
Many women do not consume sufficient amounts of folic acid through their diet.Typically, they do not eat sufficient servings of vegetables rich with folate such as beans, lentils, spinach etc without any processing such as canning or heavy cooking.

This is quite important and a very sensitive topic for me because we often see women from rural part coming to us only after completing first 4-5 months of pregnancy without any history of folic acid intake due to lack of awareness and it is heartbreaking to deliver a child with a neural tube defect and to witness the emptiness and hopelessness on parents face.
So please spread this information as much as possible.

Let's discuss your timeline for antenatal visits!
In recent years ,I realised that patients’ trust in their doctors and healthcare is declining and the consequences go beyond physicians’ lowered status.
In my clinical practice ,I often feel that patients have even started doubting when we ask them to pay frequent visits during last trimester or in complicated cases saying that doctor want to make more money or even when we ask them to come after almost a month during first trimester in uncomplicated cases stating that doctor doesn't have time for us.

So today I wanna discuss about how often one should visit your doctor during pregnancy and do my teeny weeny part of improving the doctor patient relationship.

As per WHO minimum 4visit(even this has increased to 8 in the recent WHO guidelines) are required during pregnancy but that's for people in rural area with no medical facility in the vicinity but for others who are blessed to have medical facility at their doorstep,
Your first visit should start prior to conception following which next visit should be soon after conception.
In first trimester if everything goes fine,your doctor will call you once in 4 weeks following which once in 2weeks in 2nd trimester and weekly in 3rd trimester to keep a count on your Blood pressure,baby heart beat and any other symptom which you might develop during last few weeks.
But remember this is just the overview,your doctor might call you more often if you have high Blood pressure or derranged sugar or some other complication.

So all you pregnant ladies out there,trust your doctor,be regular with your visits,make a list of all your pregnancy doubts and discuss it with your doctor,don't hesitate to have a second opinion if you have to... but please don't rely on what other people at home or your friends or what internet has to say

So couple of you guys asked me to elaborate more on DHA.
So here it is...DHA is omega 3 fatty acid that helps in brain growth and cognitive development.
Before, during, and after pregnancy, DHA is an essential nutrient for mom and baby.
Infact one study showed that maternal DHA status in pregnancy is positively associated with infants’ problem solving skills and cognitive behaviour.

Do we need to have extra DHA?
Well...Maternal DHA levels progressively decline during pregnancy thus, unless compensated.. stores will be deplete
*

Now coming to the most important question...what to consume to have that extra DHA??
Seafood and dietary supplements are the primary sources of DHA (but don't forget to consult your doctor before taking any supplements in pregnancy)

And it is also said that moms have better mood when they consume DHA during pregnancy.
So all you soon to be daddy out there..you know what to feed your wife to stay away from the side effects of those unpredictable mood swings;) and ofcourse! don't forget to grab some for yourself too because we all need to include this in our diet to have a healthy and better living!

Congratulations on completing your first trimester!
From now on,I can assure you that it's going to be more comfortable ride for you.
With Vomiting, nausea slowly going away, appetite increasing day by day, baby belly showing up and oh ya! Feeling the first baby movement..you surely going to feel the magical power of motherhood!

Remember to eat right, throughout!
You should take your iron and calcium supplements regularly but supplements are just not enough.The main aim is to improve your dietary habits.You should eat iron and calcium rich diet(infact here we usually ask patients to cook in iron pots),try to take good amount of protein along with diet rich in DHA because that helps in development of baby.
Eat lots of vegetables,pulses,egg,meat(home cooked),have milk regularly,try to include coconut water,fruits,salads in your diet.
The average weight gain in 2nd and 3rd trimester is around 450-500gm/week and for that you don't have to take more than 300-400 extra cal/day.

You will have all kind of cravings but remember that pregnancy is a diabetogenic state and even if you don't have any history of diabetes,try to keep a count on your sugar intake.

*
The growing baby will compress your stomach so you can have regurgitations and that can cause lot of discomfort so make sure to walk for about 20minutes after every meal(unless you are asked to have complete bed rest for some other complication),have plenty of water , small and frequent meals should be your aim and most importantly don't get sucked up to your bed soon after having a late night meal.

And all lovely mothers out there, remember that most important thing that you have to feed yourself is positive thinking,
Trust me..you and your baby is doing just fine.Don't fall for what people have to say..in case of any doubt ,please consult your doctor and most importantly...have faith in her!
