Frontiers in Liver Disease (FILD) is an annual meeting jointly organized by the Integrated Liver Care Program (ILC) at Aster CMI Hospital, Bengaluru and Aster Medcity, Kochi. The sixth meeting, FILD 2019 will be held in Bengaluru on Jan 12-13, 2019.

FILD focuses on recent advances in the management of patients with chronic liver disease, cirrhosis, liver failure, and liver cancer. It also aims to apprise the practicing gastroenterologist/hepatologist on the latest developments in the field of liver transplant.

A major focus, this year, will be on case-based discussions that would aim to integrate concepts of evidence-based care and expert opinion into real-life clinical practice.

Eminent national and international faculty will be sharing their experiences in the field of hepatology, transplantation, & liver intensive care.

The target audience is practicing hepatologists, gastroenterologists, transplant surgeons and physicians and residents in training. We welcome you to participate in this meeting.

Don't ignore newborns with persistent yellowness of the body !!!
A lot of newborns develop jaundice in the first few days after birth. Jaundice is the yellow discoloration of the eyes and the body. This type of early jaundice, also known as "Physiological Jaundice," usually settles down in 15 days. In some kids, the yellowness may persist longer and this is a reason to worry. A condition called EHBA or Extra-hepatic biliary atresia is a common cause of persistent jaundice. EHBA is due to faulty development of bile ducts in the liver due to which the bile produced by the liver cannot pass into the intestines. A surgery known as Kasai's portoenterostomy, needs to be done urgently within the first 60 days. Kids who do not get diagnosed with this disease continue to have persistent jaundice and land up with liver failure in the next few months. At this stage of liver failure, only a Liver Transplant can give them long term survival. For young parents in India with a new family, funding a transplant can be challenging. Our ILC Foundation runs an affordable transplant initative helping kids get liver transplants. Hazel and Daya had liver failure due to Extrahepatic Biliary Atresia and recieved liver transplants as part of this initiative. Two years after their successful surgery, they continue to live and grow like normal kids. May God bless all the donors from all over the country who have contributed for these surgeries.

MEDICAL INNOVATION "Brush Strokes"
Children who experience chronic medical conditions like chronic liver disease and liver cirrhosis and undergo major surgery like Liver Transplantation, share a need to express how they feel. For such children, art has been shown to make a massive difference in their recovery. The Integrated Liver Care Foundation in association with Aster CMI Hospital, Bangalore collaborated with some of the finest young artists in the country to present "Rang" an art therapy session for children. Artist Praveen Kumar and his team conducted an interactive painting and drawing session and taught children different ways to express themselves through various creative tasks.

High fructose in diet usually in sweetened beverage drinks and foods containing High Fructose Corn Syrup (HFCS) has been associated with the development of fatty liver in adults and children. Use of sweetened fruit juices, carbonated sweet beverages and food containing HFCS should be curtailed as much as possible. Special emphasis should be given to the fast food diet of children that is rich in fats and sweetened beverages. Read the whole article below.

‘‘Apicius, the great Roman Chef, made the discovery in the 1st century AD, that it was possible to make fatty liver (foie gras) in goose by feeding them dried figs and dates which were a rich source of fructose, and then drenching them with wine mixed with honey"

Now we are discovering that fatty liver in adults and children may be related to the excessive intake of sugars especially fructose that is found in sugary beverages. Watch out for our detailed review on this topic ...........

2018 Theme for International Nurses Day - Nurses A Voice to Lead – Health is a Human right.
Nurses are the real force behind the recovery of patients. It is we, physicians, who know how inefficient our therapies would be, were there no good and dedicated nurses. On our page, I would like to thank all the wonderful nurses who work in our team and as part of our Liver Transplant Program. We wish you all the success in your careers and your life. Each one of you is a Florence Nightingale.

This is another one of our champions. Broke his hand while playing cricket. Nothing can stop him from freaking out, playing and enjoying with his friends. He wants up to grow and design concept cars. Oh yes, he also had a liver transplant a year ago and then a few endoscopies and then another surgery to sort out a narrowed bile duct. But now all that is history. His life is on track. He gets no concessions from his mom, who donated part of her liver to make sure that this amazing kid will grow up like anyone else. In the group picture, you can see our young master (on his birthday) with two other kids who had liver transplants at the same time. To his right is his Mother.

Often it is difficult to make out post liver transplant patients after they recover completely. Amongst the two individuals in the centre, one was the patient who underwent a transplant and the other was his brother-in-law who donated a part of his liver. A year after the surgery, I am sure you will find it difficult to guess who is the patient and who is the donor. After a successful liver transplant life is back to "absolutely" normal. Except for medications which have to be taken regularly and for periodic blood tests, once in 2-3 months, patients live a normal social life; they can work like anyone else; participate in sports; go to public places like cinemas and shopping malls; they can travel; and they have no restrictions on food and water as long as it is clean and healthy. Essentially it is a new and healthier life. Diseases related to lifestyles such as diabetes, hypertension and heart disease will of course still occur just like anyone else and therefore it is important to maintain health, perform regular exercise and avoid weight gain. Periodic tests once a year to check for these diseases are recommended. Our Integrated Liver Care Team at Aster CMI Hospital, Bangalore, India became the first liver transplant centre in Asia to use normothermic machine perfusion to preserve livers using the OrganOx machine.

This organ perfusion system was used on a liver from a cadaveric donor. This liver was extremely fatty (it had more than 50% fat) and was deemed unsuitable for transplantation. The normothermic perfusion system helps to keep organs such as liver outside the body for a short period of time (12-24 hours). Oxygenated human blood replete with specific nutrients is perfused through the liver and special tests are done to monitor its function. If such livers, that were otherwise unsuitable for transplantation, work well while on the machine, they can be used for transplantation.

The liver, in this case, was used for an extremely sick 52-year-old man with an end-stage liver disease. The transplanted liver despite having 50% fat worked extremely well and the patient was discharged 12 days after a successful surgery.

Such machines will be very useful in countries like India where they can also be used to transport organs after retrieving from cadaver donors over long distances without compromising the quality of the organ. Normally organs can be stored in ice for up to 12 hours only.

Dr Darius Mirza from Birmingham, UK helped us in using the organ perfusion system for the first time in India. Dr Mirza is a pioneer in the development of organ perfusion systems and is part of one of leading teams in the world that have done a majority of research on such devices.
