36 yr Female pt came with history convulsion and bilateral progressive painless loss of vision.

On examination pt is drowsy , disoriented and bilateral Fundus examination s/o optic atrophy.

Hormonal studies s/o hypocortisolemia and hyponatremia.

MRI s/o Giant solid n cystic lesion extending from sellar+ suprasellar and compressing on brainstem with calcification s/o Craniopharyngioma.

Surgery plan : Pterional craniotomy with use of various corridors interoptic, optico-carotid, trans lamina terminalis and lateral to carotid..

Post-op pt had DI which controlled with medication.

Carotid endarterectomy....A rewarding surgery!! A 65 yr lady with history of recurrent stroke / TIA . On investigation found to have more than 80% blockage. Patient had undergone carotid endarterectomy with complete removal of block without any deficit. (Some intraoperative pics and postop Doppler with good ICA flow )
