50 YR female patient had open cholecystectomy (gall bladder removal) operation one year back. After the operation was done, her pain abdomen recurred.
Consulted many physicians and even got upper GI endoscopy done to rule out gastric ulcers, which came out to be normal. None advised ultrasound abdomen.
Finally she came herself for ultrasound which revealed dilated CBD with stones in distal CBD suggesting choledocholithiasis as the cause of her pain. Interestingly, she didnot have jaundice or any other features of biliary obstruction.
Ultrasound should be the first investigation of choice in case of any pathology related to abdomen. In case if ultrasound findings donot corroborate with clinical history, repeat scan should be advised after certain time period (depending on case scenario).
