Part 1 : This video shows how to create custom shaped CAIRS that is personalized for each patient using the Jacob CAIRS customizer. A simple, quick, accurate and elegant technique with great results to quickly and accurately transfer the custom parameters including shape based on the custom shaped plan that is created for each patient (not shown here) to the CAIRS segment that is to be implanted. Pseudophakic bullous keratopathy with decentered single piece PMMA IOL - operated yesterday. The same IOL is translocated into a glued IOL. Care needs to be taken while translocating single piece PMMA IOLs to not fracture the fragile haptics. The pupil is made circular and centered using SFT, vitrector and endodiathermy. The PDEK graft is from a 36 year old donor and thus has advantages of young donor transplantation.

Custom shaped trephined for best results! Avoiding a corneal transplantation, even if DALK, definitely gives not just a lot of satisfaction but also safety and better outcomes to both the surgeon and the patient. Note the very normal post op topographic values as compared to pre-op where Kmax was 78.3D! This patient would definitely have been a candidate for DALK in the pre-CAIRS era!

Immense privilege to have collaborated with such an esteemed set of editors with huge knowledge of the Refractive Surgery field! It was a pleasure to write the chapter on Complications of SMILE! Can't wait to read the other chapters - all contributed by a wonderful group of colleagues!

Huge congratulations to Dean Ouano and Michael Mimouni for innovating the lovely MICE technique for Corneal neovascularization! We adopted this technique after reading their article and found good regression of blood vessels as seen in the image attached. Grateful to Mimouni for guiding me personally in my first cases! Just seeing our article published online confirming our positive experience using their innovative technique!

Nostalgia...
Sharing a video on rhexis that I made for my for Playlist on my YouTube channel more than 10 years back. With close to about 1.7 lakh views so far, I've always had someone or the other come up to me at a conference just to say how the video was helpful to them. For those who haven't seen it, sharing it here hoping it helps even more beginners!

Below is my description of 10 years back :) :
In the first of this series- "Phaco for beginners" - that I am planning, I demonstrate certain simple tips and tricks for performing a rhexis in a simple manner for beginners. As you get more and more accustomed to making a rhexis, you will find that certain variations from this technique might feel better in your hands. Feel free to make changes that suit you. Remember, change is the only constant! Nevertheless, basic principles remain the same, so go ahead and enjoy!! :-)
