COSMETIC GYNAECOLOGY

Cosmetic gynaecology is an upcoming sub speciality in the field of gynaecology. The area of special focus not only includes cosmetic procedures to enhance the aesthetic appearance of the vulvo- va**nal region, but also the functional va**nal repairs to enhance or help to restore s*xual function following the changes that may occur due to child birth and / or with aging.

Cosmetic gynaecology fields include two aspects—
1. Non- surgical procedures
2. Surgical procedures

Non- surgical procedures are carried out with the help of Lasers, RF ( Radio Frequency), HIFU or HIFEM machines. These are non invasive, OPD base procedures and anaesthesia is not needed.

The most commonly performed non-surgical procedure in cosmetic gynaecology is in Urinary Incontinence and it is carried out by Laser or HIFU. Urinary Incontinence is a very common problem in the post- menopausal age either due to some hormonal deficiency or following multiple normal va**nal deliveries wherein the bladder neck becomes weak. Thus mild Urinary Incontinence symptoms can successfully be treated with the help of Laser or HIFU.

The next common problem is va**nal laxity. Again this is due to post menopausal status or following normal va**nal deliveries. This is due to some amount of va**nal laxity which again can be treated with Laser or HIFU.

Other problems which can be solved with the help of non- surgical procedures are Post partum toning of pelvic floor after va**nal delivery, Dry va**na, Lichen Sclerosis in post menopausal age, Vaginismus, Dark skin of v***a, decreased Libido or Stress mark reduction etc.

Surgical or Invasive cosmetic procedures include surgeries like Labioplasty, Hymenal reconstruction, va**nal tightening , TOT for severe Urinary Incontinence etc.

Cervical Cancer

Cervical cancer is the commonest ge***al cancer in our country and second most common cancer in women. It is caused by human papilloma virus (HPV) which is s*xually transmitted etiologic agent. The most common subtypes, HPV 16 and 18, account for about 70% of cervical cancer.

Cervical cancer is a preventable condition as this disease have characteristic features like; it has long pre-invasive stage, have some good screening tests and it can be treated well in the pre-invasive stage. The peak age of incidence of pre-invasive stage is 25-35 years while that of invasive carcinoma is 35-55 years which means that it takes about 10 years for the disease to progress from pre-invasive to invasive cancer. If cervical PAP smear screening is done every 3 years, then pre- invasive cancer can be detected and treated with 100% success.

Screening recommendations—

< 20 years— No screening recommended

21-29 years— PAP smear alone every 3 years

30- 65 years— HPV and PAP smear co-testing every 5 years ( preferred) or PAP smear alone every 3 years ( acceptable)

> 65 years— no screening recommended if adequate prior screening has been negative and high risk is not present

HPV is supposed to be the most important etiologic agent for this disease and now with the advent of HPV vaccine which is available against the two most important oncogenic strain (16 and 18) of the virus, the disease can be prevented to the extent of 70%. To be most effective it is to be given to the girls between 9 years to 12 years. Catch up vaccination is recommended upto the age of 26 years but efficacy declines.

Some modifiable risk factors like lower age at first in*******se, multiple s*x partners, too many pregnancy, poor hygiene, non use of condom etc. can be avoided to prevent cervical cancer.
