The hypothalamo-pituitary-adrenocortical (HPA axis) is required for stress adaptation. Activation of the HPA axis causes secretion of glucocorticoids, which act on multiple organ systems to redirect energy resources to meet real or anticipated demand.
Chronic stress-induced activation of the HPA axis takes many forms (chronic basal hypersecretion, sensitized stress responses, even adrenal exhaustion), with manifestation dependent upon factors such as stressor chronicity, intensity, frequency and modality.
Chronic stress leads to reduced sensitivity of the negative feedback system that governs the hypothalamic-pituitary-adrenal (HPA) axis. The loss of this negative feedback system is due to an increased level of circulating glucocorticoids (GCs). HPA axis dysregulation results in downstream physiological consequences, increasing risk for immune system dysfunction, mood disorders, metabolic disease, and cardiovascular disease.

Many people suffer from any or all of the common musculoskeletal disorders (MSDs), including pain in the neck-shoulder region, lower back, and upper extremities, or more widespread pain, that generally remain undiagnosed, or not appropriately treated for long.

Most of these chronic pains are sympathetically maintained and didn't respond to conventional ways of pain management. Symptomatically patient deteriorates with time and many other lifestyle diseases develops. Various behavioral issues, Gastric motility issues, and other musculoskeletal issues started troubling the patient.

Management is Early pain control followed by prevention of recurrence of pain. For prevention of recurrence appropriate pain management, a short course of medication, postural and lifestyle modifications, and regular stretching exercise are required.
Pain is easy to manage if our approach is good, so adopting a correct and aggressive approach at an early stage is the key to success.

Something most important, that is affecting our physical & mental health and troubling us in day-to-day activities; our posture, which we often ignored. Rehabilitation in sports is the restoration of optimal form (anatomy) and function (physiology). It is a method created to enhance functional capacity, fitness, and performance while minimizing the loss associated with acute injury or chronic disease.

The rehabilitation process should begin as soon as possible after an accident, and it should be integrated into a continuum with other therapeutic interventions including the use of pharmaceutical medications.

The athlete’s goal to return to the same sport and setting where the injury happened must be taken into account in the rehabilitation strategy. Biomechanical correction helps in preventing recurrent injuries.

Integration of early injury management and biomechanical correction is very important for any athlete. With time we will learn about our mistakes. We are managing most of the lifestyle problems as the disease and inviting so many side-effects because of medicines.

Lifestyle problems can be managed with lifestyle modifications and not as the disease for which we need lifelong medicines. Yes, uncontrolled symptoms required medicines but not the mild to moderate symptoms.

What are common side effects of common Hypoglycemic drugs are:

1. Hypoglycemia (Low Blood Sugar): This is the most significant risk associated with many hypoglycemic medications, particularly sulfonylureas and other insulin secretagogues. Symptoms may include dizziness, sweating, confusion, and irritability.

2. Gastrointestinal Issues: Medications like Metformin can cause gastrointestinal side effects such as nausea, diarrhea, and abdominal discomfort, especially when starting treatment or increasing the dosage. These effects often improve over time.

3. Weight Gain: Some hypoglycemic drugs, particularly sulfonylureas and thiazolidinediones, may lead to weight gain, which can be a concern for individuals managing type 2 diabetes.

4. Skin Reactions: Allergic reactions or skin rashes can occur with certain medications, although these are less common.

5. Other Rare Side Effects: Some medications may cause more severe side effects, such as liver function abnormalities, joint pain (notably with DPP-4 inhibitors), and fluid retention or heart failure (with thiazolidinediones).

These common side effects again managed with medicines only, so instead of treating a lifestyle issue, we are making ourselves vulnerable to many diseases. Look for cure instead of converting a lifestyle issue into a disease.

Sports injuries or injuries occurred during sports activities need early and targeted treatment. Most of the time it occurred because of our faulty biomechanics and early and proper correction not only help us in early healing but help prevent us from recurrent injuries.

Most of the time we are clueless about our injury management and wasted too much time in recovery. During this period our body loss muscle strength and elasticity much faster than a normal person, so it is very important to start sports activities as early as we can.

We are working on the basic concept of healing, if we correct the cause we can prevent the recurrence, so biomechanical correction is the most important step in the healing process. Steroids are having so many side effects, but is it really better to take NSAID instead of trigger point injections?

Nonsteroidal anti-inflammatory drugs (NSAIDs), including both traditional nonselective NSAIDs and the selective cyclooxygenase (COX)-2 inhibitors, are widely used for their anti-inflammatory and analgesic effects. In that way they are just treating the symptoms and not the cause. Means we are keeping disease with ourselves instead of looking for cure.

"Myofascial Pain" and its associated symptoms are very difficult to manage, because there are so many different theories about this pain. Because most of the patient's thought it is because of either brain or spine related issues, it is often misdiagnosed or improperly treated. Erroneously used medicines like "Duloxetine, TCA, Anticonvulsants and anti-depressants generally aggravating the psychiatric symptoms over the period of time.

Trigger point injection followed by exercises is one of the simple and effective way of treatment, mostly effective after single cycle, but in few cases repetition is required. Above all, it helps in preventing us from psychiatric manifestations of pain and negative mood.

One of the most common causes for neck and low back pain has to do with our poor and stressful posture. Because the majority of conditions in neck and low back pain falls under the category of chronic repetitive overuse injuries, and a poor posture increases the load/pressure on the ligaments, joints, muscles, and tendons trying to absorb this repetitive load. In other words, poor posture will predispose and/or increases the risk of developing a repetitive overuse injury in the neck and low-back region.
The reason for this is that poor posture will lead to the development of a system of muscle imbalances and overcompensation patterns (unstable patterns) in our body. “Over the time, this imbalance will spread throughout the musculoskeletal system in a predictable manner.”
Postural syndromes are very common in western societies. Our lifestyles such as sitting behind a desk for hours on a daily basis, daily driving, prolonged use of smart phones and texting, poor diet, poor ergonomics, lack of exercise due to time constraints, and not paying attention to our slouching contribute significantly to developing postural syndromes. To decrease this phenomenon the least we can do is to be a bit more aware of our posture and learn proper ergonomics while sitting behind a desk. It only takes a small effort to make significant changes.

Things are moving really fast, with time our body is becoming vulnerable, and we are more susceptible to infective, lifestyle diseases and malignancies.

Why it is happening and are we really moving in a right direction. Since last 2-3 decades our dependency on medicines is increasing drastically but are we getting cure or just taking symptomatic treatment only. Why we are becoming so weak with time.

Lifestyle changes and dependency on electronic gadgets is making us more vulnerable to many diseases, above all our understanding of the disease process is decreasing with time. Instead to biomechanical correction we are going for surgical interventions, instead of cure we are looking for symptomatic relief. That shows our weakness, out reduced tolerance and lack of understanding of the disease.

Each specialty teaches us how to treat a disease and what the proper approach, but we are now straightaway touching the extreme, instead of using stepladder.

Posture and Faulty biomechanics are two of the most common but neglected factors, responsible as an initial trigger for most of the lifestyle and other diseases. But, because of our faulty understanding we are showing more faith in medicines. It's better to wake up now and start improving our biomechanics and posture.

Myofascial pain induced sympathetic overactivity (Storm) is responsible for increase muscle stiffness, generalized muscle soreness, fatigue and tiredness.

Chronic of sudden acute para-spinal spasm can lead to straightening of spine and increased risk of intervertebral disk bulge/protrusion, particularly in vulnerable groups (Congenital Joint hyperlaxity, Vitamin D3 deficiency, prolonged sitting). So, it is very important to first rule out the potential treatable causes of Neck & Back Pain.

Comprehensive Treatment (Pain management) followed by posture correction and stretching exercises are always required for the recovery of functions and cure from the disease.

Most of these pains are either postural or adaptive, so proper posture care and stretching exercises are always recommended. Regular Posture Analysis is a good method to assess progress. It is a simple and harmless procedure to assess your progress.

Most of the time postural aberrations are either adaptive secondary to chronic pain or faulty habits. Relief from pain and postural correction followed by regular stretching & strengthening exercises can not only prevent us from disabilities but also prevent us from many life-style diseases.

Management is simple and effective, only identification & understanding of the disease process is lacking. Most importantly identify the cause, take appropriate treatment and live a healthy and pain-free life.
