Steroids are having so many side effects, but is it really better to take NSAID instead of trigger point injections?

Nonsteroidal anti-inflammatory drugs (NSAIDs), including both traditional nonselective NSAIDs and the selective cyclooxygenase (COX)-2 inhibitors, are widely used for their anti-inflammatory and analgesic effects. In that way they are just treating the symptoms and not the cause. Means we are keeping disease with ourselves instead of looking for cure.

"Myofascial Pain" and its associated symptoms are very difficult to manage, because there are so many different theories about this pain. Because most of the patient's thought it is because of either brain or spine related issues, it is often misdiagnosed or improperly treated. Erroneously used medicines like "Duloxetine, TCA, Anticonvulsants and anti-depressants generally aggravating the psychiatric symptoms over the period of time.

Trigger point injection followed by exercises is one of the simple and effective way of treatment, mostly effective after single cycle, but in few cases repetition is required. Above all, it helps in preventing us from psychiatric manifestations of pain and negative mood.

One of the most common causes for neck and low back pain has to do with our poor and stressful posture. Because the majority of conditions in neck and low back pain falls under the category of chronic repetitive overuse injuries, and a poor posture increases the load/pressure on the ligaments, joints, muscles, and tendons trying to absorb this repetitive load. In other words, poor posture will predispose and/or increases the risk of developing a repetitive overuse injury in the neck and low-back region.
The reason for this is that poor posture will lead to the development of a system of muscle imbalances and overcompensation patterns (unstable patterns) in our body. “Over the time, this imbalance will spread throughout the musculoskeletal system in a predictable manner.”
Postural syndromes are very common in western societies. Our lifestyles such as sitting behind a desk for hours on a daily basis, daily driving, prolonged use of smart phones and texting, poor diet, poor ergonomics, lack of exercise due to time constraints, and not paying attention to our slouching contribute significantly to developing postural syndromes. To decrease this phenomenon the least we can do is to be a bit more aware of our posture and learn proper ergonomics while sitting behind a desk. It only takes a small effort to make significant changes.

Things are moving really fast, with time our body is becoming vulnerable, and we are more susceptible to infective, lifestyle diseases and malignancies.

Why it is happening and are we really moving in a right direction. Since last 2-3 decades our dependency on medicines is increasing drastically but are we getting cure or just taking symptomatic treatment only. Why we are becoming so weak with time.

Lifestyle changes and dependency on electronic gadgets is making us more vulnerable to many diseases, above all our understanding of the disease process is decreasing with time. Instead to biomechanical correction we are going for surgical interventions, instead of cure we are looking for symptomatic relief. That shows our weakness, out reduced tolerance and lack of understanding of the disease.

Each specialty teaches us how to treat a disease and what the proper approach, but we are now straightaway touching the extreme, instead of using stepladder.

Posture and Faulty biomechanics are two of the most common but neglected factors, responsible as an initial trigger for most of the lifestyle and other diseases. But, because of our faulty understanding we are showing more faith in medicines. It's better to wake up now and start improving our biomechanics and posture.

Myofascial pain induced sympathetic overactivity (Storm) is responsible for increase muscle stiffness, generalized muscle soreness, fatigue and tiredness.

Chronic of sudden acute para-spinal spasm can lead to straightening of spine and increased risk of intervertebral disk bulge/protrusion, particularly in vulnerable groups (Congenital Joint hyperlaxity, Vitamin D3 deficiency, prolonged sitting). So, it is very important to first rule out the potential treatable causes of Neck & Back Pain.

Comprehensive Treatment (Pain management) followed by posture correction and stretching exercises are always required for the recovery of functions and cure from the disease.

Most of these pains are either postural or adaptive, so proper posture care and stretching exercises are always recommended. Regular Posture Analysis is a good method to assess progress. It is a simple and harmless procedure to assess your progress.

Most of the time postural aberrations are either adaptive secondary to chronic pain or faulty habits. Relief from pain and postural correction followed by regular stretching & strengthening exercises can not only prevent us from disabilities but also prevent us from many life-style diseases.

Few of the symptoms because of acute or chronic trapezius tightness are:

1. Poor postural alignment and shoulder position
2. Upper back pain that radiates or worsens with movement
3. Lower back fatigue and reduced stability
4. Lack of arm and shoulder mobility and strength
5. Soreness, stiffness, and spasms of the trapezius muscle
6. Tingling or numbness in the arms
7. Posterior headaches
8. Myalgia or muscle pain

In simple words, trapezius tightness is responsible for most of the postural pains and neurological symptoms not only around neck but also in the upper & lower back.

Don't blame your spine for every pain, instead most of the pains are secondary to musculoskeletal problems in your body. Wrong posture is one of the major triggers for musculoskeletal pain.

Postural/Myofascial/Migraine/Tension headaches must have a common origin from a common source, and most of the time it will become generalize in a short span of time. In simple term, we must know about origin of these pains. Most of these pains are biomechanical, because of our faulty biomechanics.

Pain is generally not severe, associated with giddiness, gastritis and behavioral changes. In most of the cases, it is chronic in nature with waxing and waning course. But, pain is associated with endocrinological & metabolic changes and with time with predominant autonomic symptoms.

It is always good to control pain early and prevent secondary endocrinological & metabolic issues.

In can be managed and controlled quite easily in a short span of time with various pain management procedures. Pain relief is followed by a proper stretching and post procedure care to prevent recurrence. There's a no need to take medicines for long duration of time.

Knock knee (also called "knock-knee deformity", "knocked knee" or "genu valgum" is an incorrect alignment around the knee that can affect can people of all ages.

Knock knee is a condition in which the knees bend inward and touch or "knock" against one another, even when a person is standing with their ankles apart. This places excessive force on the outer side of the knee, which can cause pain and damage over time.

It is most common in children, typically affecting 3- to 6-year-olds. In most young children, the condition will naturally correct itself by age 7 years. In rare cases, genu valgum may continue into adolescence. In other cases, older children or adults may develop it as a result of an underlying disease or condition.

Knock knee is usually bilateral – affecting both legs – but in some cases, it may only affect one knee. Correction of the deformity will improve knee mechanics and walking capability, reduce pain and prevent a rapid progression of damage to the knee.

Treatment for genu valgum will largely depend on the cause and severity of the condition. Biomechanical correction works best if done appropriately and aggressively. Wait and watch policy never works for any disease, so early aggressive management is the key to success. It takes time but results are mostly good.

Maintaining good tone and shoe modification should be started early for best results.

Myofascial Pain (MP) is generally associated with a constellation of symptoms besides pain & discomfort. Dysautonomia or Sympathetic storming is one of the most important components of chronic MP.

Dysautonomia is a potentially life-threatening syndrome seen in many different types of brain injuries or because of chronic myofascial pain. It involves paroxysmal sympathetic hyperactivity and typically includes a constellation of symptoms, including tachycardia, tachyarrhythmia, tachypnea, hyperthermia, hypertension, diaphoresis, muscle hypertonia, and/or decerebrate or decorticate posturing.

Long-standing dysautonomia can precipitate and make us vulnerable to many lifestyle issues like Hypertension, Diabetes, Thyroid disorders, Metabolic syndrome, and many Musculoskeletal issues. Recurrent episodes of Dysautonomia can even precipitate stroke, neurodegenerative disorders, and many psychiatric issues.

Early management is the only cure. Both injectable and non-injectable options are available, and the problem can be cured easily. Ultimately, a healthy and pain-free body is what we need in our routine life.

Maintaining a fine balance between the two divisions of Autonomic Nervous System (ANS) is very important for a healthy living. We often ignore the origin of any life-style disease and often start focusing on the symptoms. But, for any secondary life-style disease there should be a cause, and by simply treating the cause we can prevent ourselves from almost all the life-style diseases.

We often ignore the existence of latent Myofascial triggers in our body and tries to suppress the symptoms arising out of it instead of looking for a cure. These latent triggers are responsible for sustained higher Sympathetic Nervous System (SNS) tone in our body and this high SNS tone is an initial trigger for many life-style illness like Diabetes, Hypertension, Thyroid Disorder, Metabolic Syndrome and Chronic pains.

We can simply avoid these by timely treatment of these myofascial triggers. This requires an early aggressive approach followed by regular stretching exercises and supplement intake. No need to take regular analgesics. It's always good to look for cure instead of just suppressing the symptoms.

Now-a-days as we are performing most of our activities on hard floor, we are increasingly forcing our feet into overpronation. Overpronation happens when your gait (the way you walk or run) eventually causes the arches of your feet to flatten more than they would normally. Overpronation increases the risk you’ll injure your foot, leg, knee, hip and back.

It is now very common to see overpronation because most of our activities involves hard surface. With repeated activities on hard surfaces and subsequent weakness of intrinsic muscles feet aggravates the risk of flatfeet, knee & back injuries and reduced sports performance.

It's always better to undergo periodic biomechanical assessment to arrest and correct these issues on time so that unnecessary medication and surgical intervention can be avoided.

We can't ignore the fact that with our modern lifestyle and increasing dependency on electronic gadgets, heavy loads of studies and ignorance about posture is pushing our next generation towards an early and heavy burden of various lifestyle diseases.

We are now progressing towards the "Primitive Posture" i.e. flexed or folded posture. We are not aware, but we are destroying our future generation. They are forced to live under the heavy loads of lifestyle diseases at an early age. But we are so much ignorant about the posture and myofascial pain that we are now becoming more vulnerable with time.

By adopting faulty posture, we are just increasing subclinical level of stress in our body, and this low-level stress is keeping our "Sympathetic Nervous System" (SNS), tone towards higher side. And this high SNS tone is the starting point for almost all the lifestyle diseases in our body.

Just don't ignore your child's posture, if posture is not good, it may be starting point of any lifestyle illness.

Mind your posture and live a healthy and pain free life.
