What changes occur in the body as we age?

A wide range of changes can happen in the body to different degrees as we age. These changes are not necessarily indicative of an underlying disease but they can be distressing to the individual.

Even though the aging process cannot be stopped, being aware of these changes and adopting a healthy lifestyle can reduce their impact on overall health.

Expected bodily changes of aging include change in:

Skin: With aging, skin becomes less flexible, thinner, and more fragile. Easy bruising is noticeable, and wrinkles, age spots, and skin tags may become more apparent. Skin can also become more dry and itchy as a result of less natural skin oil production.

Bones, joints, and muscles: Bones typically lose density and shrink in size making them more susceptible to fractures (breaks). Muscles shrink in mass and become weaker. Joints can suffer from normal wear and tear; joints become inflamed, painful, and less flexible.

Mobility and balance: A person's mobility and balance can be affected by various age related changes. Bone, joint, and muscle problems listed above in conjunction with changes in nervous system are the major contributors to balance problems. Falls may occur resulting in further damage with bruises and fractures.

Body shape: As a result of bony changes of aging, body stature can become shorter and curvature of the back vertebrae may be altered. Increased muscle loss and reduced fat metabolism can also occur. Fat can redistribute to the abdominal area and buttock areas. Maintaining an ideal body weight becomes more difficult.

Face: Aging changes also take place in the face. Other than wrinkles and age spots, the overall facial contour can change. Overall loss of volume from facial bone and fat can result in less tightness of the facial skin and sagging. The face becomes droopier and bottom heavy.

Teeth and gums: Teeth can become more weak, brittle, and dry. Salivary glands produce less saliva. Gums can also recede (pull back) from the teeth. These changes may result in dry mouth, tooth decay, infections, bad breath, tooth loss, and gum disease.

Hair and nail: Hair can become thinner and weaker as a person ages. Dry hair may lead to itching and discomfort. Nails may become brittle and unshapely. Nails can also get dry and form vertical ridges. Toe nail thickening (ram's horn shape) is common. Nail fungal infections may occur frequently.
Hormones and endocrine glands: Hormonal changes are seen commonly in the elderly. Most common is the hormonal control of blood sugar and carbohydrate metabolism leading todiabetes. Thyroid dysfunction and problems with fat and cholesterol metabolism are also commonly encountered. Calcium and vitamin D metabolism may also become altered. Sexual hormones reach a low level and can lead to erectile dysfunction and vaginal dryness.

Memory: Problems with memory are common in seniors. However, it is important to realize that minor memory problems do not constitutedementia or Alzheimer's disease. Simple lapses of memory such as not remembering where you left a key or whether you locked the door are a normal part of aging.

Immunity: The body's immune system can get weaker with age. Blood cells that fight infections (white blood cells) become less effective leading to more frequent infections.
Hearing: changes in nerves of hearing and ear structures can dim hearing and cause age-related hearing loss. Higher frequencies become harder to hear.

Vision: Eyes can become drier and the lens can lose its accuracy as we age. Vision can be affected by these changes and can become blurry and out of focus. Glasses or contact lenses can help correct these problems.

Taste and smell: Sense of smell and, less commonly, sense of taste may fade leading to poor appetite and weight loss.
Bowel and bladder: Bowel and bladder control can cause problems with incontinence (involuntary loss of f***s or urine). Additionally, bowel and bladder habit can change. Constipation is common in older adults, as are urinary frequency and difficulty initiating urine.
Sleep: Sleep patterns can significantly change with age. Duration of sleep, quality of sleep, and frequent night time awakening are commonly seen in seniors.

DIETARY GUIDLINES FOR DIET RELATED CHRONIC DISORDERS ADVOCATED BY BINAPANI GERIATRIC CARE

Eating wrong type of foods can lead to several problems in old age. Obesity, hypertension and heart disease as well as diabetes are common diet related degenerative diseases of old age. Simple diet restrictions will help to not only control these disorders but also if started early enough may even prevent the onset of these chronic ailments.

1. In case of Obesity:-
Obesity is basically accumulation of excess body fat leading to an increase in body weight. obesity is not only unactractive, but it leads to many other problems like arthritis, heart diseases, diabetes, etc. Hence maintaining weight within normal range will keep one healthy and active. Some dietary considerations to reduce weight include:

a) Decrease the amount of cereal foods eaten eg: rice, chapati, bread
b) reduce intake of fatty food like ghee, butter, oil, etc.
c) avoid food with excess sugar
d) do some form of physical exercises.

2) In case of Diabetes Mellitus
Diabetes Mellitus leads to an increase in blood sugar levels. This disease is due to a deficiency of insulin in the body. Obesity is a strong predisposing factor. Though there is no cure for diabetes, it can be controlled by suitable dietary interventions. If uncontrolled , it can lead to several complications affecting the eyes, blood vessels and Kidneys. Controlling weight is the first step in the management of this disease. other dietary considerations include:

a) avoid sweet foods
b) restrict the use of starch foods
c) include whole cereal and pulses
d) eat vegetables.

private concern. The current need is to conduct research and explore new directions in research on population ageing. Also, sub national networks can be established for the research. These networks can facilitate exchange of information, resource sharing and training opportunities. Such networks may be developed by the relevant national agencies in conjunction with appropriate persons at the International level to ensure optimal cooperation and success of research endeavors. There is a need to reorient health sectors toward managing chronic diseases and the special needs of the elderly. All the stakeholders have reached consensus on the need to educate and train healthcare providers in geriatrics to address issues of provider shortages, distribution, education, and quality as well as training in elderly care. As pointed by Dey et al., the key challenges to access and affordability for elderly population include reduced mobility, social and structural barriers, wage loss, familial dependencies, and declining social engagement. Community volunteers and medical, paramedical, and indigenous providers must be trained to identify issues common among elderly patients, to conduct comprehensive surveys of morbidity and functional status, and to engage in capacity building of geriatric services in local communities. Multidisciplinary approaches to geriatric care can be fostered by collaborative training among medical, paramedical and other professionals. Future care giving for the elderly will also require models of both formal and informal care and systems for supporting caregivers. Disability significantly affects quality of life in old age. Caring for the elderly in a way that addresses disability and maintains good quality of life has become a global challenge. Considering this there is an urgent need to train more professional caregivers to focus on elder care in order to meet current and future demand. The same has been urged by World Health Organization (WHO) in its publication towards and International Consensus on Policy for Long- Term Care of the Ageing

Geriatrician Overview

A geriatric physician, also called a geriatrician, is a medical doctor who specializes in the diagnosis, treatment, and prevention of disease and disability in older adults. Geriatric physicians are primary care doctors who are specially trained in the aging process.

Geriatricians provide comprehensive medical care for people later in life. They are board certified, usually in either family medicine or internal medicine, and have received additional training and certification in geriatric medicine.
Geriatric physicians are concerned with health problems that frequently affect older adults, such as pain, falls, memory loss, incontinence (involuntary loss of urine), and medication side effects. Another important aspect of geriatric medicine is evaluating an elderly patient's ability to care for him- or herself (e.g., prepare and eat meals, bathe,DRESS ).
Medical conditions that geriatricians may treat include the following:
• Alzheimer's disease
• Cancer (e.g., ovarian cancer, prostate cancer)
• Dementia
• Diabetes
• Epilepsy
• Heart disease
• Osteoporosis
• Parkinson's disease
• Sleep disorders
• Stroke

Due to improvements in medical care and nutrition and advances in technology, the number of people in the United States who are over the age of 65 is larger than ever before. This growing population often has special medical concerns.
Primary care physicians (e.g., general practitioners, family practitioners, internists) may provide medical care for elderly patients who do not have serious health concerns. However, when an older adult experiences physical, mental, or emotional illnesses or disabilities that lead to a dependence on assistance from others, or when his or her condition causes stress on caregivers (e.g., family members and friends), a geriatric physician should be consulted.
Geriatric physicians focus on all aspects of health care and work as part of a health care team to address issues that affect the elderly. Medical professionals who work with geriatricians include nurses, social workers, nutritionists, physical therapists, occupational therapists, pharmacists, and geriatric psychiatrists. The geriatric health care team focuses on many aspects of the patient's life, including his or her social support (e.g., spouse, children, other relatives), living conditions and home life, and community.

Geriatricians provide care in a number of settings. They see patients in hospitals, nursing homes, rehabilitation centers, clinics, and physician offices, and often provide or manage in-home care.
