A 71y old male presented with sudden onset of slurrish speech, vertigo, dementia and generalized weakness. At ER, his GCS was E2 V2 M3 and Blood pressure was 150/60mmHg, and on examination, Bilateral crepitation +, hypovolemic and deranged electrolyte profile. Initially conservative management with broadspectrum antibiotics, Bronchodilators with nebulization has done. He has a previous history of Alzheiemer's disease, Parkinsonism, Hypertension and Type II diabetes mellitus. Investigation seen EEG in Normal limit, Serum Na+ (very low) 128, K+ 2.8, Hb 11.8, TLC 11,800, MRI Dementia revelas no significant study whereas HRCT thorax seen bilateral infiltration and ?Bilateral effusion. What is the likely diagnosis?
