Low sodium is a frequent problem widely present in admitted sick patients.mostly asymptomatic but depending upon the rapidity thats less than 48 hours(acute). It causes brain to swell giving rise to headache,altered behaviour weakness,nausea and vomitting and if not treated coma and convulsions.For quick diagnosis both blood and urine osmolarity and urine sodium helps in differential diagnosis.there are three types 1depletional

Treatment depends upon acuteness.
Rapidity falling sodium needs hypertonic 3% saline and gradual correction.whereas avoiding free water intake to less than a litre or 500 ml is the treatment for chronic(more than 48 hours old) low sodium and various combinations of salt supplimentation diuretics and fluid restrictions is tried.
Some times especially euvolemic(siadh) low sodium requires treatment with drugs like vaptans.
