The anterior communicating artery (ACom) lies deep in the brain at a complex junction of vital vessels.

Aneurysms in this location demand meticulous planning, refined microsurgical skills, and absolute respect for surrounding perforators and blood flow.

Microsurgical clipping allows direct visualization and precise sealing of the weak point, ensuring durable protection while preserving normal circulation.

This Neuro-vascular surgery demands experience, judgment, and precision.

Cerebral cavernous malformation located in the dominant hemisphere motor cortex — an eloquent brain region where millimeters matter.

Pre-operative MRI demonstrates the lesion, and post-operative imaging confirms complete excision with preservation of surrounding cortex.

In such eloquent areas, intraoperative ultrasound (iUSG) offers real-time, dynamic localization, accurate depth perception, and continuous updating despite brain shift, which can significantly limit static neuronavigation accuracy after dural opening.

Current literature supports iUSG as a safe, reliable, and cost-effective guidance tool for resection of small, cortico-subcortical cavernomas in eloquent cortex, aiding maximal safe resection while minimizing neurological morbidity.

Precision is not optional in eloquent brain surgery.
Real-time guidance makes the difference.

🧠 A complex, giant ruptured aneurysm of the right Middle Cerebral Artery (MCA) — managed microsurgically.

The pre-operative angiogram showed a large, multilobulated aneurysm arising from the M1–M2 bifurcation with compromised distal flow.
The post-operative angiogram demonstrates complete clipping and preservation of distal branches, marking a successful surgical outcome.

a balance between life and precision. 🔬

My Friend, a great Neurosurgeon, Dr. Jagath lal and Myself with Prof. Hirotoshi Sano, a legend in cerebrovascular neurosurgery and one of the world’s foremost experts in aneurysm surgery. His pioneering work in microsurgical clipping and multi-clip strategies for complex aneurysms has set benchmarks in our field.

Emeritus Professor at Fujita Health University and a lifelong mentor to countless neurosurgeons, Prof. Sano continues to inspire with his dedication, skill, and commitment to teaching.

A privilege to stand beside a true giant in neurosurgery. 🙏🧠✨

🧠 Cerebellopontine Angle Tumor – Completely Excised with Facial Nerve Preservation

The left image shows a contrast-enhancing tumor at the left cerebellopontine (CP) angle, compressing the brainstem and cerebellum — classic for a vestibular schwannoma, also known as an acoustic neuroma. These tumors arise from the vestibular division of the 8th cranial nerve and often present with hearing loss, imbalance, or facial numbness.

🎯 Surgical management was done via a retrosigmoid suboccipital craniotomy using microsurgical dissection under high magnification.

⚡️ Intraoperative neuromonitoring (IONM) was employed throughout the procedure to continuously monitor facial nerve function, enabling safe dissection and preventing nerve damage.

📈 The post-operative MRI (right) shows complete tumor excision with no residual lesion and beautifully decompressed brainstem.

👇 And most importantly, the patient is doing well — fully alert, facial nerve function completely preserved, no deficits, and recovering smoothly.
