This is a baby born to a mother who was brought to Umri Christian hospital unbooked. Her water bag broke 24 hours ago, she was getting mild pain and her bp readings were very high (160/100 mm Hg). She was in obstructed labour ( baby was stuck in the birth passage) and baby’s heartbeat was very low ( 90-100 beats per minute). Also baby’s head was posterior in the birth passage (occipitoposterior). With so many complications and high risk factors our gynaecologist made a swift decision to do an Emergency Caesarean section. Caesarean was done and thankfully baby cried at birth 😊. There were complications during the surgery also which can happen in obstructed labour but they were managed by our doctor single handedly. Now both mother and baby are doing well.

82 year ,Mrs.Sakubai resident of our old age home in Mure Memorial Hospital ,evaluated and diagnosed to have a large epigastric hernia.Pre op investigations and echo was done for pre anaesthetic evaluation.After taking into considerations all the risks involved with surgery and anaesthesia in the peri-operative period ,she was planned for open epigastric hernia mesh repair.She was operated by upper GI surgeon.Thoracic epidural was placed for post op analgesia.Intra op period was uneventful and she was shifted to icu post extubation for monitoring.However 24 hours post op she developed respiratory distress and her oxygen requirement increased.After discussion and opinion of physician and intensivist and surgeon, she was investigated and started on started on prompt treatment some of which included diuretics, nebulizations and steroids .Regular incentive spirometry and chest physiotherapy was continued by our physiotherapist .She was ambulated on post op day 1.Analgesia was taken care of, by epidural. She didnt require any opioids post op for her pain management.Over days her oxygen requirement reduced .She was finally off oxygen on post op day 12 .Her medications were optimised .She was educated about using MDIs and discharged on post op day 15

This baby’s mother was brought to MMH in active labour. She was unbooked(had only one antenatal visit), never took iron or calcium supplements throughout pregnancy. She came with severe anaemia (hemoglobin of 7 gm/dl)and her bp was shooting up to 160/110. We immediately transfused blood and gave medicines to bring down her bp. To add to this, baby passed meconium (stools) in the tummy. Our obstetrician conducted a forceps (instrumental) delivery to save the baby and also made sure that the mother doesn’t lose much blood. Three days later, the mother and baby were discharged in a healthy condition. The mother totally received three bottles of blood and was started and bp control medicines.

Moral of the story: Please go for antenatal checkup’s and please take iron and calcium tablets regularly.

Our orthopaedic surgeon operating on a distal humerus multifragmentary intraarticular fracture (lower arm/ elbow fracture). He has done open reduction and internal fixation via posterior approach and olecranon osteotomy. 90-90 plate configuration with medial and posterolateral plates were used (in simple terms fixed the fracture with plates).
