Dharma Foundation of India Our projects in India are centred around health promotion in community ,taking into consideration Universal Health Coverage and Health for All.

Health and social services delivered within a neighbourhood by local people in local establishments, and community-based support and voluntary groups can play an important role in delivering support and care to vulnerable people and older persons.The Self-Management and Community Program will not conflict with existing programs or treatment as it is designed to enhance regular treatment and disease-specific education given by clinicians/family physicians in healthcare organization. It may form a cost effective way to introduce prevention of co morbidities, education and information centres in elders.Systems that integrate and coordinate health and welfare programmes will be both more effective and efficient in addressing the type of health care needs of many persons. Integration is a basic requirement both in and among health and welfare sectors. The community care centre (CCC) is proposed as a means of spreading awareness on the social issues and primary healthcare approach to better address the needs of older adults and vulnerable populations . The proposed approach provides a framework for integrating health and welfare systems and strengthens peoples ‘connections to their family and communities regardless of abilities or disabilities”

*Step 2: Educational Sessions*

- Focus on practical, culturally appropriate health education that’s easy to understand.

*Step 3: Community Involvement*
- Promote peer support and local community participation to encourage engagement.

- Track progress and collect feedback from participants regularly.
- Use findings to adapt and improve the programme content and delivery.

Distributed health literacy :

Traditionally ,health literacy is seen as an individual trait focussing on a person's ability to understand and use health information.

However ,distributed health literacy recognises that health knowledge is often shared across families , communities and networks .
Support from others -whether carer explaining medications ,a community leader bridging cultural information and communication gaps ,or a peer educator sharing health messages - strengthen collective health literacy.

People with strong health networks experience better health outcomes and improved confidence in managing their care and finding their way around the healthcare system.
