Unexpected result for a seemingly difficult case. Matching a maxillary central to its fellow is indeed a very challenging scenario. If this is complicated with a thin ridge and a demanding female patient, you are certain to get a little not so confident.

A 3mm implant ( 3 x 13 )was immediately provisionalised with a screw retained chairside acrylic temporary whose margins were modified with flowable composite. Bone graft and membrane were religiously placed and fixed respectively. Flap was coronally repositioned with a classic horizontal mattress suture by Dr Anuj Aggarwal (flap closure was the turning point i feel )

Sutures were removed after a week and the patient went to here native place in arunachal pradesh for 4 months. She returned for her finals and the soft tissue was nicely plump and contoured. Promptly fabricated a screw retained layered zirconia crown which matched well.

Always heart warming to see those bone levels. 3 years follow up. Adin Swell implants. Platform switching. 4mm collared abutments. Cemented zirconia crowns.
As they say ...tissue is the issue but bone sets the tone.

Transformation with twin block n braces
In 2009 a 9 year old girl was brought in by her parents complaining she never made friends or went out because she 'looked a bit different'.
Phase 1 Twin block appliance in 2009 was followed by Phase 2 Fixed braces in 2012. She was put on retention in 2014 and has been stable since. Walked in today and i wasted no time to grab my camera.

Any Body Can Lift - ABCL

Sinus lift or sinus augmentation is a procedure not attempted by every dentist owing to a number of reasons. However ever since we have started using the MASAI or MAgnetic Sinus Artificial Intelligence Drills by Mr Curette Korea, the procedure has become more predictable. It essentially has just two round drills. One drill has serrated margins to score the bone. The second and final drill is a special drill with two mini Alnico magnets which repel each other inside the drill.(see pictures) While in contact with bone , the pressure forces the magnet together and the drill actually cuts bone. As soon as the tip or cutting edge touches the membrane the resistance is lost and the magnets repel each other, thus stopping the tip or cutting edge from perforating the membrane. In fact the round disc of bone can be reused neatly to close the window.

Dr Anuj Aggarwal has done dozens of direct lifts using these drills in the past 3 - 4 years with great success and more importantly predictability. Anybody can lift in the true sense.

It was exciting to demo the new 3Shape Trios intra oral scanner introduced by Mr Shammi ji from Unicorn. Worked well , scanned fast and was forgiving with raw bleeding margins. The casts were printed on a 3d printer with excellent 3 point articulation. Hope to buy one soon.

This case is a classic example of how to convert a simple straight forward case to a complex one and make life difficult. Done along with Dr Animesh Agarwal and Dr Manish Vishen at the formers practice.
Female patient had a root stump in the upper incisor . Impression was made followed by a suck down stent to help locate the cingulum entrance of the pilot and subsequent palatal 3D placement of the implant. Somehow the patient was an elderly female who did not like our big camera and flashes. I got distracted and the palatal inclination of the drill went off buccally. So now ended up in a situation where the implant is about 15 degrees buccal and hence a screw retained temp was out of the question. Did not want to use a cementable temp for sure specially in the esthetic zone. Thankfully i had a few samples of Adin TMA multi unit abutments which the company gave away free to me. Made indirect impressions after 10 days because of the delay in finding a solution. Used the 17 degrees TMA to get the angulation right and then fabricated a composite screw retained crown over a plastic cylinder. Used a Style Italiano shade tab from 3M as a facing for the temp. However on screwing in the metal multiunit TMA in the patients mouth, there was a dark hue through the gingiva which was not cool. Also the temp margins could hold the gingiva more for better emergence and the shade could be matched a tad better. Will improve the temp in the coming month.

Braces work !!!
A young girl was extremely embarrassed of her smile and the gap between her front teeth. In orthodontic terms she had a unilateral posterior cross bite , anterior open bite , peg lateral and a missing lateral on the other side.
We relieved her crossbite with a special unilateral expansion appliance. This appliance was also inspired by one bluegrass appliance by Haskell and Mink with a green bead in the middle. The patient was encouraged to play with the bead with her tongue which reprogrammed her tongue position and positioned it away from her front teeth. We then further closed her open bite with elastics and opened up space for her lateral. A narrow diameter implant was placed and an acrylic tooth was made to 'ride' on the wire for esthetic reasons till the implant integrated. A definitive crown was luted along with a proper retention protocol. The other peg was built up with composite for esthetics and Boltons.
I sincerely believe orthodontics can do wonders to the overall personality and self image of the growing adoloscent. Special thanks to Prof Dr U S Krishna Nayak for all the help as always.

A patient walked in at 6 pm today with a slightly mobile discolored anterior maxillary incisor and walked out exactly after 90 minutes with an implant and immediate screw retained provisional. Great team effort all round specially by my brother and surgeon Dr Anuj Aggarwal. Simple case done without too many bells and whistles with gratifying immediate predictable results.

Patient presented with an avulsed 11. Tooth was avulsed 7 days back while on vacation and patient had preserved it in coconut milk. Implant placed making sure that the emergence would be at future cingulum area to keep the option of screw retained temp open. Insertion torque acheived 15 ncm...implant size 4.2 x13 Adin Toureg. Novabone putty graft placed in the jumping distance. What made this case interesting was the outcome of temporisation. The patients own tooth was modified and bonded onto the palatal aspects of the adjacent teeth.

Patient said...its looking a little whiter than it was before doc...and im like...maybe we will patent coconut milk for bleaching...hehe lets beat Ramdev at his own game.
