An elderly gentleman with post infective sequelae in lung presenting with recurrent Haemoptysis. Was on conservative management, however there was two episodes of massive Haemoptysis today with haemodynamic instability. Emergent bronchial artery embolisation done. Post procedure patient is stable!!!

Here is a 13 year old boy with recurrent significant epistaxis. Work up evaluation with CT paranasal sinus showed Stage II Juvenile nasopharyngeal angiofibroma (JNA). Pre op embolisation is done to minimise the risk of intra op bleeding! Pre op embolisation of Juvenile nasopharyngeal Angiofibroma can reduce intra operative blood loss while lessening the risk of massive haemorrhage, shortening operation times, increasing intra operative visibility and allowing for easier resection of lesions…!!!

Case of savaging mucormycosis in post Covid status. Presentation with right sided facial swelling, retro-orbital pain & altered sensorium. MRI revealed right sided rhino-sinusitis changes with extensive involvement of ipsilateral infra-temporal fossa(ITF) & PPF with peri neural extension along maxillary division through foramen rotundum to ipsilateral cavernous sinus with thrombosis of cavernous segment of ipsilateral ICA. There is an intracranial lesion of probable fungal etiology in right temporal lobe as well. Patient had to undergo extensive debridement!!!

A case of savaging Rhino-orbital cerebral mucormycosis (ROCM) in a post Covid case. Presentation of right sided facial swelling, ipsilateral retro-orbital pain & altered sensorium. MRI revealed rhino-sinusitis with involvement of right infra-temporal fossa ( ITF) and PPF extending perineurally along foramen Rotundum to ipsilateral cavernous sinus and thrombosis of cavernous segment of ICA. Involvement of greater wing of sphenoid is also there. There was a focal intracranial lesion in right temporal lobe as well, likely fungal etiology. Patient had to undergo extensive debridement.

Acute Stanford type B Aortic dissection with the dissection flap extending to right renal artery & resultant mild right renal hypoperfusion! Caudally the dissection flap is extending all the way up to left external iliac artery! Patient needs strict BP control and Aortic stent grafting for its management!!!
