Anesthesia in leprosy patient-Tips:

1.History and examination is important

R/O symptoms of ANS disturbances
R/O neuropathy
R/O drug reaction
L/F skin patches,deformities(airway and peripheral),myopathy

2.Bedside tests:
Valsalva
Orthostatic hypotension
PFT-BHT,SBCT

3.Investigations:
L/F peripheral smear,CBC-for pancytopenia,agranulocytosis
LFT,RFT
NCS
Skin scrapings
G6PD deficiency

4.Challenges:
General Anesthesia-difficult airway,drug metabolism ,m**h hemoglobinemia
Regional Anesthesia-new deficits
Intra op-hypotension and urinary retention

5.Dapsone must be stopped 5-7 days before Sx.

-Now sternum in line with EAC

Epidural Anesthesia and Elderly:

-Changes in epidural space with age

1.Decrease in fat content
2. Sclerosis at intervertebral foramina
3. ⁠increased compliance and decreased resistance of the space

All these changes lead to faster onset and greater cephalad spread of LA

-Physiological beta blockade—epidural test dose not reliable

-Non linear relationship between LA dose requirement per segment

-Biphasic absorption of LA
Initial rapid absorption due to increased vascularisation
Slow absorption phase due to epidural fat
